Case Manager - Job Description
Description: Oshki-Giizhig is an Indigenous-based non-profit organization. our vision is a healthy community built on Indigenous journeys towards wellness. Strengthened by cultural practices and traditional teachings, support will assist in restoring balance to individuals and the community affected by trauma and violence. Qualifications: Post-secondary education in social work, social justice, Indigenous studies, health related and/or a combination of pertinent experiences in these areas. Experiences with cultural activities and understanding of indigenous concepts of care and support. Experience leading a team in a dynamic fast-paced environment. Knowledge of disabilities, disorders and mental health issues.
Driver’s License and access to transportation.
Responsibilities The successful applicant will meet with participants, their families, and relevant collaterals to provide an introduction to our program and collaboratively develop individualized support plans. The role is responsible for maintaining, reviewing, and updating participant support plans on an ongoing basis, and regularly communicating relevant information to staff to ensure coordinated service delivery.
This position ensures participants have access to appropriate health, cultural, and community-based services, and works collaboratively with internal teams and external systems identified within each support plan. The successful candidate will actively connect participants with resources to support their individual needs and goals.
In addition, this role involves community engagement through the provision of transportation services, including driving participants to and from appointments, programs, and other required locations to support their full participation in services.
