The Primary Health Care Nurse Practitioner will conduct comprehensive health assessments and physical examinations across the lifespan, using evidence-informed clinical reasoning to formulate diagnoses. The role involves collaborating with clients and interdisciplinary care teams to develop and coordinate individualized care plans, as well as identifying trends in client health needs to inform program planning and advocacy efforts. The incumbent will also be responsible for maintaining clear, accurate, and timely documentation, including electronic medical records, reports, and requisitions.

Qualifications
- Current registration with the College of Nurses of Ontario as a PHCNP
- Proof of Liability Insurance
- Minimum of five (5) years of clinical experience in a healthcare role, ideally in primary care or community health, demonstrating readiness for advanced practice as a Nurse Practitioner.
- Proficiency in Microsoft Office and electronic medical record systems
