The Chronic Disease Management (CDM) Cardiac Rehabilitation Nurse utilizes the nursing process, critical thinking and problem solving skills to provide care to clients across the life span in the area of chronic disease management. The CDM Cardiac Rehabilitation Nurse ensures the development, implementation and ongoing administration of the client care plans. The CDM Cardiac Rehabilitation Nurse assesses, coordinates, supervises, administers and evaluates the provision of health services and healthcare resources. The CDM Cardiac Rehabilitation Nurse accepts responsibility for continuing competencies through ongoing professional development, including participation in education programs, research and continuous quality activities. The CDM Cardiac Rehabilitation Nurse demonstrates the ability to accurately and effectively assess client status and to take appropriate intervention measures that have significant impact on the client's medical condition, safety, as well as ultimate recovery. The CDM Cardiac Rehabilitation Nurse may provide education and support in the areas of diabetes, weight management, cardiac risk reduction and lifestyle management.
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
Additional Required Qualifications:
Experience in cardiovascular prevention care and management and cardiac rehab. License(s)/Certificate(s)/Professional membership(s): AHS RHA requires that health services provide be registered as a regulated member by their applicable College and that the health services provider provide proof of their registration upon request of AHS RHA. The College of a profession provides the ability to work in Full Scope of Practice, including restricted activities that are authorized by the regulations of the profession.
Preferred Qualifications:
Bachelors’ Degree in a related Health discipline. Equivalent combination of education and experience will be considered.
