Position Title: One-to-One Community Support Worker
Position Type: Part-Time
Position Reports To: Home Share Manager
Schedule: Monday to Friday – 20 hours per week (4 hours per shift) – Evenings (Generally 6-10pm) with occasional afternoon activities
Wage: Per Collective Agreement. JJEP Grid 10. $27.53/hour.
Job Summary
Following the person-centered plan, the Community Inclusion Worker (CIW) provides one-to-one support to a designated individual so they may access and participate in meaningful involvement in their community. The CIW assists the client with activities of daily living, and plans, coordinates and implements social, recreational, educational, and vocational activities and programs specific to the needs and goals of the designated individual.
Duties & Responsibilities:
- Participates in assessment, goal setting, and program planning such as Person-Centered Plans for the designated client.
- Identifies social, economic, recreational, physical, vocational, and educational services in the community that will meet the client’s needs.
- Assists and encourages the client to participate more independently in programs, activities, and activities of daily living. Assists with daily life skills, social skills, and/or behaviour management.
- Assists in planning client schedule and routine.
- Plans, organizes, and facilitates physical, recreational, education, social, and vocational activities to meet the client’s goals and needs.
- Assists with activities of daily living such as feeding, lifts, transfers, hygiene, grooming, and toileting where applicable.
- Recognizes, analyzes, and manages potential emergency situations such as client’s aggressive behaviour, to minimize potential harm to client and/or the public.
- Reports concerns and problems to the program manager.
- Accompanies and/or transports clients to activities such as appointments, shopping, or leisure activities.
- Provides written and/or verbal reports regarding clients’ daily activities and progress to the Program and ensure all required documentation is complete and accurate.
- Liaise with family and community, caregivers, professionals, and organization staff.
- Completes in-house learning modules on Open Futures Learning.
- Performs other related duties as required related to the 1:1 position.
Qualifications and Requirements:
- Class 5 B.C. Driver's License is required.
- Access to a reliable vehicle is required. Mileage will be reimbursed.
- Non-Violent Crisis Intervention Certificate or equivalent.
- Satisfactory ICBC Driver's Abstract.
- Clear Criminal Record Check for the purpose of working with vulnerable adults.
- Valid Level 1 First Aid and CPR.
- Physically and mentally able to perform the duties of the job (Certificate of Fitness for Job Duties).
- WHMIS and FoodSafe.
- Ongoing professional development through Open Future Learning.
- Ability to work effectively with staff, management, and clients.
- Able to meet applicable legislation and regulations (i.e., Community Care and Assisted Living Act, PHO Mandates).
- Must be able to participate in moderate to high level of physical activities (adaptive hiking, kayaking, pushing a wheelchair during long walks within the community, swimming, etc.).
- Demonstrated ability to work independently.
- Must be willing to use public transit when appropriate.
- May be required to use lifts to transfer the client.
- Will be required to provide personal care to the client.
Preferred Education:
- · Diploma in related human / social service field.
Preferred Training and Experience:
- One (1) year of recent, related experience working with adults with an intellectual/developmental disability.
An equivalent combination of education, training, and experience.
