Licensed Practical Nurse (LPN) – Home Monitoring Program (Full/Part-Time)
Location: Surrey, BC (12578 72 Ave, Surrey, BC V3W 2M6)
Naz's Pharmacy is seeking compassionate and motivated Licensed Practical Nurses (LPNs) to join our Home Monitoring Program. Our team is dedicated to helping patients live healthier, more independent lives by delivering high-quality nursing care in the comfort of their own homes.
With over 200 patients and a team of 40+ healthcare professionals, Naz's Pharmacy is committed to providing exceptional community-based care through a collaborative and patient-focused approach.

Position Details
- Part-time
- Weekday and weekend availability required
- Statutory holiday availability required
- Morning Shift: 7:30 AM – 2:00 PM
- Evening Shift: 3:00 PM – 9:00 PM
- Shifts range from 5–8 hours
- No overnight shifts
- Driver's license and reliable vehicle required
- Mileage reimbursement provided for work-related travel
About the Role
As an LPN in our Home Monitoring Program, you will visit multiple patients during each shift to provide medication-focused nursing care in their homes.
This role does not involve personal care, activities of daily living (ADLs), or patient transfers. Your primary focus will be medication administration, patient assessments, monitoring health status, and collaborating with the pharmacy and healthcare team to ensure safe, effective care.
Key Responsibilities
- Conduct comprehensive head-to-toe assessments
- Monitor and document vital signs, weight, and overall health status
- Administer medications including:
- Oral medications
- Inhalers
- Eye and ear drops
- Creams and ointments
- Transdermal patches
- Intramuscular (IM) injections
- Subcutaneous injections
- Support diabetes management, including:
- Blood glucose monitoring
- Insulin administration using sliding scales/protocols
- Recognition and management of hypoglycemia
- Monitor medication compliance, including witnessed medication administration where required
- Recognize changes in physical, mental, and emotional health and report concerns appropriately
- Develop and evaluate individualized nursing care plans
- Collaborate with pharmacists, physicians, patients, families, and other healthcare professionals
- Maintain accurate electronic documentation in real time
- Update patient logbooks and communicate changes in care promptly
- Promote patient independence through education and support
- Maintain a safe and clean care environment
- Participate in orientation and mentorship of new team members
- Attend ongoing education and professional development sessions
- Perform other related duties as required
Qualifications
- Current practicing registration with the British Columbia College of Nurses and Midwives (BCCNM)
- Valid Class 5 driver's license with a clean driver's abstract
- Reliable vehicle
- Criminal Record Check (required upon successful offer)
- Professional references
- Previous LPN experience preferred (minimum 1 year preferred)

- Comfortable working independently in the community
- Proficient with electronic documentation, Microsoft Outlook, and Google Workspace
