Responsibilities
- Assess patient health status through comprehensive evaluations and develop tailored care plans that meet individual needs.
- Administer medications, treatments, and injections accurately following physician orders and established protocols.
- Monitor patient progress, document clinical findings, and adjust care strategies accordingly to promote recovery and well-being.
- Educate patients and their families on disease management, medication usage, and lifestyle modifications to foster independence.
- Coordinate with multidisciplinary teams using electronic health records systems such as Epic or Cerner to ensure seamless communication and continuity of care.
- Conduct specialized procedures including phlebotomy, pain management interventions, and dementia or senior care techniques.
- Maintain detailed documentation of all patient interactions, treatments administered, and clinical observations to ensure compliance with healthcare regulations.
Requirements
- Valid RN license in good standing with relevant state nursing board.
- Proven experience in hospital settings with acute care exposure; home care experience is highly preferred.
- Familiarity with electronic health record systems ; medical terminology proficiency required.
- Strong background in patient care across diverse populations including pediatrics, seniors, and individuals with dementia or assisted living needs.
- Demonstrated skills in medication administration, laboratory procedures like phlebotomy, and pain management techniques.
- Excellent communication skills for effective case management and patient education.
- Ability to work independently while collaborating effectively within multidisciplinary teams.
