Overview
We are seeking compassionate and dedicated Home Care Providers to deliver personalized in-home care to clients in need. This role is vital in supporting individuals with varying health conditions, disabilities, or age-related needs, ensuring they maintain independence and quality of life. The ideal candidate will possess a genuine desire to assist others, excellent communication skills, and a strong sense of professionalism. Prior experience in assisted living, nursing homes, hospice care, or working with seniors and individuals with disabilities is highly valued. Duties
- Assist clients with daily activities such as bathing, dressing, grooming, and mobility support
- Provide medication reminders and administer medications as prescribed
- Prepare nutritious meals tailored to individual dietary needs
- Perform light housekeeping tasks including cleaning and laundry to maintain a safe and comfortable environment
- Support patients with mobility challenges through safe transfer techniques and assistance with walking or exercises
- Monitor health status and report any changes or concerns to healthcare professionals or family members
- Offer companionship and emotional support to promote mental well-being
- Assist with specialized care needs such as dementia care, hospice support, or caring for individuals with disabilities
- Follow care plans accurately and document services provided during each shift
- Works at a clients Home (living in)
Skills
- Prior experience in assisted living, nursing homes, hospice care, or senior care is preferred
- Strong patient care skills with a compassionate approach toward clients of all ages and conditions
- Knowledge of medication administration and safety protocols
- Ability to perform moderate lifting when necessary for client transfers or mobility assistance
- Excellent communication skills for effective interaction with clients, families, and healthcare teams
- Experience working with individuals with disabilities or dementia is advantageous
- Capable of performing light cleaning and meal preparation to ensure a clean and healthy environment
- Reliable, punctual, and able to work independently while following detailed care instructions
- Comfortable working with and maintaining pets
- Additional home based activities where necessary including vacuuming and some gardening
