PROGRAM SUPPORT MENTOR
Purpose and Summary
LYC is a non-profit community-based organization that has been supporting adults living with mental health illness. We work collaboratively with our funders, our partners and other stakeholders, across the provincial health and social service systems, to inspire positive change in the way that services are delivered to individuals living with mental health illnesses.
The Program Support Mentors are responsible for working with clients to achieve goals as outlined in their individualized Wellness Plan. The position requires respect for, and expertise working with people who face barriers to live independently due to their mental illness. The position requires someone who has demonstrated experience and skills being personable, dynamic and creative in their work with clients, co-workers, supervisor, and community partners.
Duties:
- Provide support and programming assistance to adults living with mental illness, that focuses on learning life skills and enhancing overall quality of life
- Plan, organize and facilitate daily programming activities
- Support individual clients with development of coping and grounding skills for distress situations and symptom management
- Work as a team member and advocate with community resources, medical professionals and other support services to respond to clients needs
- Assist individual clients to administer medications as directed by medical professionals arrange/provide transportation for clients
- Provide crisis intervention services as needed; including follow-up services until crisis situation has stabilized
- Actively monitors, documents and reports on progress being made by clients and and completes all logs and designated case documentation prior to leaving shift
Requirements:
- Post secondary education with studies concentrated in the field of Human Services or other related experience preferred
- Demonstrated success with assisting adults living with mental illness
- Demonstrate ability to work collaboratively as part of a multidisciplinary team
- Able to work with minimal supervision
- Excellent organizational and time management abilities
- Competent with Microsoft programs
- Available to work flexible hours / 12 hour shift rotations – days / nights / weekends
- Criminal Record Check with Vulnerable Sector Check required
- Valid Driver’s License and a reliable vehicle preferred
- Valid First Aid/CPR Certificate or willingness to obtain within first 3 months
- Valid ASIST – Applied Suicide Intervention Skills Training Certificate or willingness to obtain within first 3 months
- Valid Non-Crisis Intervention Training Certificate or willingness to obtain within first 3 months
Job Type: Full-time
Pay: $18.72-$19.87 per hour
