Position Purpose:
The purpose of the Life Skills Worker position is to improve client well-being and support better health outcomes by helping individuals and families strengthen practical life skills, maintain daily routines, access needed services, and follow through with health-related plans developed with the Nursing Department and broader care team.
The role focuses on prevention, practical support, early intervention, continuity of care, and relationship-building. It is especially important for clients who may struggle with organization, self-care, family responsibilities, emotional stress, daily living tasks, or maintaining stable connection to health services.
Job Duties/Responsibilities:
Client Support and Life Skills Development
- Provide one-to-one practical support to clients in developing and maintaining essential daily living skills.
- Support clients in building routines related to sleep, meals, hygiene, appointments, family responsibilities, and household management.
- Assist clients in strengthening organization, time management, and follow-through on personal goals.
- Encourage independence and self-confidence while providing respectful and non-judgmental assistance.
- Help clients improve life stability through practical coaching and supportive guidance rather than doing tasks for them wherever appropriate.
- Reinforce healthy living practices in line with nursing guidance and care goals.
Support to the Nursing Department
- Work closely with nursing staff to support client care plans and identified wellness goals.
- Assist in implementing non-clinical aspects of nursing care plans through regular follow-up and practical support.
- Help identify barriers that may affect client health outcomes, such as missed appointments, poor routines, social isolation, or unstable living conditions.
- Report observations, emerging concerns, and client progress back to the nurse or supervisor in a timely and professional way.
- Support continuity of care by helping clients remain engaged with nursing services and follow recommended plans.
- Participate in case coordination and help nursing staff understand practical challenges affecting clients in daily life.
Home Visits, Field Work, and Outreach
- Conduct home visits, wellness checks, and field-based support as assigned and approved.
- Provide practical support to clients in real-life settings where issues affecting health and daily functioning are most visible.
- Support outreach to vulnerable clients who may have difficulty attending appointments or maintaining regular connection with services.
- Assist with appointment reminders, accompaniment where approved, and follow-up after visits.
- Identify environmental or practical risks and communicate relevant concerns to nursing staff or leadership.
- Maintain professional boundaries, safety practices, and respectful conduct during field work.
Cultural Safety and Relationship-Based Practice
- Deliver services in a culturally safe, trauma-informed, strengths-based, and respectful manner.
- Demonstrate sensitivity to community values, Indigenous ways of knowing, family systems, and lived realities.
- Support client dignity, autonomy, trust, and meaningful engagement.
- Work in a way that respects the role of Elders, family, and community in wellness and healing.
- Help create a service environment where clients feel safe, heard, and supported.
Qualifications
- Certificate, diploma, or relevant training in community support, social services, health care support, human services, life skills work, or a related field
- Minimum 1 to 2 years of experience in community support, health support, Indigenous community services, home support, or related client-facing work preferred
- Experience working with vulnerable individuals, families, Elders, or clients with complex life circumstances
- Valid Class 5 driver’s license and reliable transportation for home visits and outreach work
- Current First Aid and CPR, or willingness to obtain
- Clear criminal record check, including vulnerable sector check
Preferred
- Experience working in First Nations, Indigenous, rural, or community-based settings
- Knowledge of trauma-informed practice, harm reduction, cultural safety, and relationship-based support
- Experience supporting clients with mental health concerns, substance use, chronic illness, parenting needs, or housing instability
- Familiarity with community resources, referral systems, and health-related service coordination
