At the Silver Willow Supportive Living site in Nanton, we provide personal and professional care services in Designated Supportive Living for clients with needs that fall within the psychosocial, physiological, health related behavior, and environmental domains. Supportive Living (SL) is recruiting a Registered Nurse in a role of a Case Manager to coordinate the placement, assessments of ongoing referral needs, care planning (both complex and annual), and delivery of professional and personal care services to our clients. Note: the Silver Willow Lodge is located in Nanton - approximately 90km South of Calgary - but it is a beautiful drive!
Description:
As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. Additional Required Qualifications:
Excellent organizational and time management skills - be able to efficiently organize many details in a time efficient manner. Minimum of 5 years current nursing experience. The successful candidate will work independently in a supportive living setting. Physically able to perform all assigned tasks. Able to sit for long periods, occasionally walk long distances and to be able to carry 25 lbs. Ability to be on the phone or work on the computer for long periods of time. Computer proficiency – Excel, Word, MS Office. Strong computer and technology skills (or ability to quickly learn and adapt) is required due to electronic documentation and care planning requirements for Supportive Living. Strong negotiation and mitigation skills; ability to remain professional and composed during times of adversity. Excellent verbal and written communication skills required. Ability to work independently with minimal supervision required. Ability to conduct (clinically competent) and have experience with comprehensive physical assessments. Ability to problem solve and communicate abnormal findings within appropriate time frames based on a risk assessment. Community or continuing care (S/L, LTC) related experience required.
Preferred Qualifications:
Baccalaureate degree in nursing is preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Geriatric and dementia experience/certification is an asset. Palliative experience is an asset. Experience with Connect Care and Netcare systems and interRAI experience. Case Management experience. Leadership skills an asset.
