Job Overview:
Reporting to the Health Director, the Community Health Nurse will work to their full scope of practice in community, providing safe, quality nursing care to patients, delivering health education programs and providing consultative services regarding issues relevant to nursing practice in accordance with the established standards, policies and procedures in Fort McMurray #468 First Nation.
Responsibilities:
- Assess patients to identify appropriate nursing interventions
- Serve as a custodian of health and access NetCare or other EMR to input patient information
- Collaborate with members of an interdisciplinary health team to plan, implement, co-ordinate and evaluate patient care in consultation with patients and their families
- Liaise with community physicians and allied health professionals to ensure effective patient care is provided
- Provide health education and registered nursing care through home visits, consult & assist in complex home care cases, participate in community needs assessment and program development, conduct disease screening and deliver immunization programs
· Implement support services which include screening and assessment of pregnant women and new parents to assess family needs, reproductive and preconception health promotion
- Monitor, assess, address, document and report symptoms and changes in patients' conditions
- May supervise licensed practical nurses and health care aides
- May teach and counsel patients and their families on health-related issues in collaboration with other health care providers
· Provide latest program information and updated statistics as required
· Attend scheduled staff meetings and perform other duties as required
Requirements:
· Bachelor of Science in Nursing (BScN) or Bachelor of Nursing (BN)
· Must provide proof of active registration with the College & Association of Registered Nurses of Alberta (CARNA) and maintain registration throughout the term of employment.
· Current Basic Cardiac Life Support-Health Care Provider (BCLS-HCA) is required.
· Valid First Aid/CPR Required
· Wound Care knowledge an asset
· Experience working in an Indigenous Community including knowledge of First Nations Culture, traditions and values is considered an asset
· Public Health nursing experience including immunization, Healthy Beginnings, and Child Health Clinic preferred
· Other desirable recent nursing experience includes maternal/postpartum and newborn assessment.
· Must possess strong assessment, communication, organizational, problem solving, and counselling skills.
· Must be able to work both independently and as a part of a team and possess a good understanding of the determinants of health.
· Effective communication skills, oral and written, are required
· Able to work positively, effectively and cooperatively within an interdisciplinary team.
· Knowledge of computer applications, including basic word processing, email, electronic charting, internet access, and data entry required
· Has or willing to obtain a class 4 drivers license
· Ability to work on-call on alternating weekends
· Successful candidate will be required to travel to Fort McMurray #468 First Nation daily
