Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to provide exceptional home care services to diverse patient populations. Duties
- Provide professional nursing care in clients' homes in accordance with individualized care plans.
- Perform health assessments and monitor changes in client condition.
- Administer medications, injections, and prescribed treatments within the LPN scope of practice.
- Complete wound care, catheter care, ostomy care, and other nursing procedures as required.
- Educate clients and families on health management, medications, and disease prevention.
- Maintain accurate and timely documentation using electronic or paper health records.
- Collaborate with physicians, Registered Nurses, case managers, and other healthcare professionals.
- Follow infection prevention and control practices and all Alberta health and safety requirements.
- Promote client independence while ensuring safe, compassionate, and person-centred care.
Skills
- Proven experience in nursing home, hospital, or acute care settings with a focus on patient-centered care
- Strong knowledge of medical terminology, medication administration protocols, and pain management techniques
- Experience working with diverse populations including children, seniors, hospice patients, and those requiring dementia or assisted living care
- Excellent case management skills with the ability to coordinate complex patient needs effectively
- Familiarity with home care environments, medical office procedures, and laboratory testing processes
