Position Overview:
The Thamesview Family Health Team (TFHT) provides primary health care, including chronic disease management and health promotion and disease prevention, to a rostered population of patients in the Chatham-Kent region. The FHT utilizes an interdisciplinary team of health care professionals who work in a collaborative model of care.
The Pharmacist will play an integral role in the care of the patients of the Thamesview Family Health Team. The Pharmacist will assist the FHT in achieving its vision and mission. The Pharmacist will support patient care and the delivery of programs including chronic disease, palliative care, cancer care, mental health and geriatric care. The Pharmacist will also provide physicians and Team providers with pharmacological consultation, education and information that contributes to the health and wellness of patients and the knowledge and expertise of providers.
Roles and Responsibilities:
Diagnosis and Treatment
- Accept all patients referred by the FHT physicians and/or interdisciplinary health providers, in accordance with the FHT's established referral protocol, to identify and resolve medication related problems and to contribute to treatment and care plans.
- In consultation with primary care providers, prepare therapeutic and management plans with patients.
- Monitor and follow up pharmacotherapeutic treatment plans.
- Document all patient care activities in the patient's electronic chart.
- Collaborate with family physicians to improve medication management for chronic diseases such as hypertension, diabetes, dyslipidemia, and heart failure.
- Participate as a clinical team member in the development and delivery of targeted programs including but not limited to palliative care, cancer care, chronic disease, mental health and addictions.
- Share results of assessment and intervention with referring physicians and other Team clinicians as requested within a specified timeframe and in accordance with established protocol.
- Apply self-management and other strategies to support the development of an informed activated patient.
- Develop, update and maintain education resources for patients and for providers.
- Provide physicians and team members with current information and guidelines on new drugs, contraindications, therapeutic use of drugs, and other information to assist in the provision of quality, evidence-based care to patients.
- Participate in individual and group education sessions for patients and providers.
- Evaluate and document outcomes of individual patient care in collaboration with the team.
- Facilitate referrals to other services as well as communication and networking between the primary care practice and hospital or home care to ensure smooth transition for medication related care between care sites.
Organizational:
- Assist in the planning, implementation and evaluation of chronic and health promotion and illness prevention programs and services including, but not limited to medication management and anticoagulation.
- Establish policies to allow for consistent and uniform procedures regarding management of medications that have been discontinued or had indications or contraindications significantly change so as to impact on patient care.
- Recommend improvements to the medication use process at the practice site(s) to enhance safety and efficiency (eg. handling of samples, administration of medications, documentation, etc).
- Collection of data as required for statistical/reporting puroses.
- Provide timely and informative reports as directed and requested by the Executive Director/Physician Lead.
- Observe, collect and record patient data in the EMR system.
- Participate in team building, program development, and improvement activities.
- Participate in staff meetings, to exchange ideas, receive mutual support and monitor program outcomes as well as present case studies.
- When required, assist in the development and implementation of the clinical model and guidelines for collaborative care.
- Comply with established personnel policies, procedures, standards and guidelines for operation.
- Identify gaps and needed modification in services.
Education, Community Relations and Advocacy:
- Facilitate referrals to other services as well as communication and networking between primary care and hospital or home care.
- Maintain relations with networks, related professional organizations and other related service providers.
- Participate in building creative supportive environments for FHT staff and for patients.
- Represent the mission, values and philosophy of the FHT in the community, including networks, meetings or presentations, as requested.
- Assist in training and supervision of students and/or volunteers as requested by the Executive Director and/or Physician Lead.
- Help patients to identify and use health resources.
- Involve patients in the decisions about their own health.
- Support patients in learning how to self-manage care.
- Initiate and support health education activities.
- Identify community needs and resources.
- Provide clinical leadership.
Other:
- Participate in established performance evaluation process and professional development goals.
- Develop, update and maintain education resources.
- Attend relevant conferences, workshops and other in-service events as approved.
- Participate in community meetings as required.
- Perform other related duties as assigned by the Executive Director and/or Physician Lead.
Qualifications and Requirements:
- B. SC (pharmacy) and registration in Part A of the Ontario College of Pharmacists.
- Community or Hospital Pharmacy residency or equivalent clinical experience in either setting is required.
- Additional training or credentialing in pharmacotherapy is considered an asset (eg. Certified Geriatric Pharmacist, Certified Diabetes Educator, Certified Asthma Educator).
- Demonstrated clinical skills in assessment, program planning, implementation and evaluation.
- Minimum one year of demonstrated experience.
- Work effectively, both independently as part of an inter-disciplinary team.
- Proven flexibility and ability to adapt to change.
- Computer skills.

Pay: $88,000.00-$99,919.00 per year
