The Opportunity
Reporting to the Program Coordinator, the Site Team Lead plays a dual role as both a direct support professional and the day-to-day leader of the participant's support team. Working alongside Community Residential Workers (CRWs), the Site Team Lead provides individualized, person-centred support while promoting consistency in service delivery, mentoring team members, and ensuring the participant's goals, routines, and support strategies are implemented consistently and effectively.
This position serves as the primary resource for the participant's support team, fostering collaboration, coaching staff, supporting communication, and promoting high-quality services that align with individualized support plans and MCL Masonview Homes' standards.
Position Overview
The Site Team Lead is responsible for providing direct, individualized support to participants receiving services from MVH while overseeing the daily delivery of participant support activities. This position combines hands-on participant care with leadership duties, ensuring the consistent implementation of support plans, fostering a collaborative team environment, and upholding high standards of professionalism, documentation, and participant-centred practices.
Working closely with the Program Coordinator, Community Residential Workers, healthcare professionals, families, guardians, and other approved members of the participant's support network, the Site Team Lead helps create a safe, stable, and empowering environment where participants can achieve their goals and live as independently as possible.
Key Responsibilities
Participant Planning & Support
- Provide individualized, person-centred support aligned with the participant's individualized service plan.
- Promote independence in daily living, decision-making, and community participation.
- Support personal wellness, appointments, household management, recreation, transportation, and daily routines.
- Recognize participant strengths and facilitate opportunities for skill development and increased independence.
- Identify participant needs, concerns, or emerging risks and collaborate with the Community Residential Coordinator to implement appropriate follow-up.
- Maintain participant rights, dignity, privacy, and confidentiality at all times.
Team Leadership & Coordination
- Lead the day-to-day coordination of supports for the participant, promoting consistency across all shifts.
- Provide leadership, mentorship, and coaching to Community Residential Workers assigned to the participant's support team.
- Support the orientation, onboarding, and ongoing development of new staff.
- Foster a collaborative, respectful, and accountable team environment.
- Promote consistent implementation of individualized support plans, behaviour support strategies, and participant routines.
- Monitor the quality and consistency of participant supports and provide constructive feedback to team members.
- Support effective communication between shifts and ensure important participant information is shared appropriately.
- Assist with identifying operational or participant support concerns and collaborate with the Community Residential Coordinator to develop solutions.
- Serve as a role model by demonstrating professionalism, person-centred practice, and MCL Masonview Homes' values.
Collaboration & Community Engagement
- Collaborate with the Program Coordinator to ensure participant supports remain aligned with changing needs and organizational standards.
Operations & Professional Practice
- Support the implementation of organizational initiatives, quality improvement activities, and participant-specific best practices.
- Assist with scheduling considerations and staffing continuity as requested by the Scheduling Manager.
- Promote accountability, professionalism, and consistency within the participant support team.
Documentation & Administration
- Review documentation practices with team members to promote consistency, completeness, and accuracy.
- Ensure participant documentation reflects current supports, incidents, goals, and outcomes.
Qualifications & Experience
- Diploma or degree in Human Services or a related field, or equivalent combination of education and experience.
- Minimum of 2–3 years of experience supporting individuals with intellectual disabilities, physical disabilities, mental health challenges, or complex support needs.
- Previous experience providing mentorship, coaching, acting as a shift lead, or informal leadership is considered a strong asset.
- Strong understanding of person-centred planning and positive behaviour support.
- Excellent communication, leadership, conflict resolution, and organizational skills.
