Overview
We are seeking a dedicated Community Occupational Therapist or a Physiotherapist to join our healthcare team. This role involves providing specialized therapy services within community settings to improve patients functional independence and quality of life. The ideal candidate will have experience with previous work in the community. This position offers an opportunity to make a meaningful impact on diverse patient populations.
The rate on this posting is per visit completed at a patients home. Visits can range anywhere from 15 minutes - 2 hours.
Duties
- Assess patients functional abilities and develop individualized treatment plans tailored to their needs.
- Deliver therapeutic interventions aimed at improving daily living skills, motor skills, and cognitive functions.
- Collaborate with multidisciplinary teams to coordinate patient care and ensure seamless service delivery.
- Manage case documentation accurately, including progress notes, treatment plans, and discharge summaries.
- Educate patients and their families on therapeutic exercises, adaptive techniques, and safety precautions.
- Monitor patient progress regularly and adjust treatment strategies accordingly.
- Facilitate community-based programs to promote health, wellness, and rehabilitation for diverse populations.
- Experience completing home safety assessment is considered an asset
Skills
- Experience working in the community is considered an asset
- Strong knowledge of medical terminology relevant to occupational therapy practice.
- Excellent communication skills for interacting with patients, families, and healthcare professionals.
- Ability to adapt therapy techniques to various clinical environments within the community setting.
Qualifications
- Has to be registered with the college of occupational therapists Ontario or the college of physiotherapists Ontario
- Must be certified with ADP (assisted devices program)
