JOB SUMMARY
Under the direction of the Health Manager, the Community Nurse is responsible for delivery of health programs within the organization to support clients in community access services both on and off the nation.
JOB DESCRIPTION
The Causal Community Nurse will work closely with all departments within the health team and agencies both on and off the nation. We strive to take a collaborative approach to ensure the best holistic care for clients in the community. Your role will be to assist partners with medication administration, wound care, case management, and other medical needs. Education, mentoring, and community training are key components of the position. The nurse will be expected to make informed decisions that contribute to the overall safety and wellbeing of the community.
DUTIES AND RESPONSIBILITIES
· Report to the Health Manager daily
· Support and have knowledge of programs such as community health, maternal health, mental health and addictions, and homecare.
· Case manages all clients within programs and update team lead of any changes
· Build relationships and trust within the community to support clients with services, referrals, and additional support
· Utilize nursing knowledge and skills to provide preventative, therapeutic, and rehabilitative care
· Perform independent nursing functions using established procedures, policies, protocols, and licensing standards
· Deliver patient care tasks safely and efficiently
· Collect data and statistics to support case management, care planning, and client goal setting
· Maintain communication between clients, significant others, healthcare team, and partners
· Carry out physician and nurse practitioner orders accurately
· Evaluate nursing care effectiveness and adjust plans when needed
· Document observations and interventions accurately in all charting systems
· Administer medications and monitor/document client responses
· Perform administration and documentation of prescribed injections
· Provide thorough shift reports with relevant client information
· Report client needs, changes in condition, and updates to the circle of care
· Conduct community and home visits daily for medication administration and follow-up
· Collaborate with the health team, contracted services, and on/off-reserve partners
· Mentor, educate, and train junior staff as required
· Support nursing team with additional duties and client care
· Maintain ongoing education and training to ensure safe and effective practice
· Apply critical thinking and respond appropriately to urgent or emergent situations
· Maintain continuous communication with off-reserve services to ensure high-quality care
· Promote and model culturally competent care practices
EDUCATION, SKILLS, AND QUALIFICATIONS:
· Licensed Practical Nurse (LPN) or Registered Nurse (RN)
· First Aid and CPR
· Experience working with vulnerable populations
· Transport of Dangerous Goods (preferred)
· Immunization Certification (preferred)
· Foot Care Training (preferred)
· Mental Health First Aid
· Trauma-Informed Care (preferred)
· Cultural awareness and competency (preferred)
· Understanding of cultural uniqueness within Treaty Seven Territory is an asset
· Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)
· Excellent verbal, written, and presentation skills
· Strong leadership abilities and abilities to mentor and supervise community members, youth, elders, and health staff
· Strong interpersonal, listening, and coaching skills
· Excellent project management and organizational skills
· Must be friendly, courteous, assertive, objective, and fair
EMPLOYMENT CONDITIONS:
· Clear Criminal Record Check and Vulnerable Sector Check
· Active Nursing License and Insurance
· Possession of a valid Class 5 driver’s license and a vehicle in good working order for work related travel.
WORKING CONDITIONS
· Community-based position with both office-based work and daily home visits
· Must be comfortable driving in winter road conditions, with potential wildlife encounters, including dawn/dusk driving
· Hours per Shift: 7 hours
