What We’re Looking For:
We are seeking a Follow - Up Support Worker with great attention to detail and excellent organizational skills and ability to multi-task to join our team.
What you’ll do:
- Provide the support for homeless clients to sustain and maintain private market housing, including completion of follow up assessment and all ETO requirements and other services to ensure the individual is housed and maintains housing
- Engage in weekly Intensive Case Management meetings to collaborate with the team, share information, enhance resource knowledge, identify suitable housing alternatives, and effectively manage clients facing eviction.
- Work with clients using a strength-based client centered approach to develop plans for long term stability.
- Facilitate regular support meetings with the clients and provide ongoing support based on need
- Provide the link to appropriate community resources, agencies, or government ministries
- Advocate on behalf of clients e.g. landlords, social service agencies, ministry workers and Mental Health case managers
- Follow up and contact with participating landlords on a regular basis as it relates to the tenant
- Mediate between the landlord and tenant as necessary
- Perform and/or coordinate the clean up and repair of problematic residences as identified above within guidelines and approved budgeted amounts
- Maintain complete records and statistics (ETO) following Housing First principles and intensive case management process
- Provide oral, written and/or electronic reports, case notes and correspondence as required, using ETO platform
- Commit to a co-ordinated and integrated team approach in conjunction with partner agencies to problem solve and for case co-ordination
- Assist in the development and implementation of policies and procedures
- Other duties as assigned
- All other duties as assigned by Team Leads
Qualifications
Education
2 year Diploma or equivalent. Candidates without a diploma but who have post-secondary education and relevant and related experience will also be considered.
Job Type: Full-time
Pay: $55,000.00 per year
