POSITION SUMMARY: The Health Navigator is an 18-month temporary term position dedicated to supporting community members in accessing and navigating the healthcare system. Reporting to the Home Care Supervisor and the Director of Health & Wellness, the Navigator ensures that all services are culturally safe, respectful, and responsive to Indigenous needs.
By working closely with Alberta Health Services, community health programs, and external service providers, the Navigator plays a key role in reducing barriers to care, increasing health literacy, and supporting individuals and families throughout their healthcare journeys. This role is essential in helping community members understand available health services, coordinate appointments, and advocate for culturally appropriate care while connecting individuals to vital medical, mental health, addictions, and social supports.
KEY RESPONSIBILITIES
Health Navigation & Advocacy

Assist community members in navigating the healthcare system including Alberta Health Services, hospitals, clinics, and specialty care providers.

Support clients in scheduling and coordinating medical appointments.

Advocate for culturally safe and trauma-informed healthcare services.
-
Provide accompaniment to medical appointments when appropriate.

Assist clients in understanding diagnoses, treatment plans, and follow-up care.
Care Coordination
-
Collaborate with physicians, nurses, mental health professionals, social workers, and other service providers to coordinate care.

Facilitate referrals to appropriate health programs and services.

Support discharge planning for individuals transitioning from hospital back into the community.

Connect clients to community-based programs including addictions treatment, mental health supports, and chronic disease management.
Health Education & Literacy
-
Promote health literacy by helping individuals understand their health conditions and treatment options.

Provide culturally relevant health education and wellness information.

Support community health promotion activities and workshops.

Any other assigned duties by Supervisor and Director
Community Outreach

Identify individuals who may be experiencing barriers to accessing healthcare.

Support vulnerable populations including Elders, individuals experiencing homelessness, and those facing addictions or mental health challenges.

Participate in outreach activities to increase awareness of health services.
Program Documentation & Reporting

Maintain accurate client documentation and service records.

Collect data for program reporting and funding requirements.

Monitor emerging community health trends and identify service gaps.
QUALIFICATIONS:
Education
-
Diploma or Degree in one of the following preferred:

Social Work

Nursing

Indigenous Studies

Combination of education and relevant experience may be considered.
Experience
-
Experience working with Indigenous communities and knowledge of Indigenous health issues.

Experience navigating healthcare systems or providing case management support.

Experience working with vulnerable populations including individuals facing addiction, mental health challenges, or housing instability.
Knowledge & Skills

Strong understanding of Indigenous culture, traditions, and community values.

Knowledge of Alberta Health Services programs and healthcare systems.

Understanding of trauma-informed care and culturally safe practices.

Excellent communication and advocacy skills.

Ability to build trust and maintain confidentiality.

Strong organizational and case management skills.
Additional Requirements

Valid driver’s license and reliable transportation.

Ability to travel to regional health facilities when required.
