Advanced Primary Care is seeking a compassionate, highly skilled, and motivated Registered Nurse (RN) to join our patient-centered medical home.
As a Patient-Centered Medical Home, we provide comprehensive, team-based primary care focused on continuity, accessibility, innovation, and exceptional patient experiences. Our Total Advanced Care Team (TACTeam) model brings together physicians, nurses, allied health professionals, laboratory staff, and medical office assistants to deliver proactive, relationship-based care.
The successful candidate will work closely with physicians and multidisciplinary team members to support patients through all stages of care, including chronic disease management, wellness assessments, virtual medicine, preventative care, and complex care coordination.
Key Responsibilities
Work in daily partnership with assigned primary care physicians in the ongoing assessment and management of patients
Assist with daily triage of patient needs and appointment prioritization based on urgency
Conduct virtual nursing consultations including telephone and email advice for routine medical inquiries and chronic disease management
Support Annual Wellness Assessments and Well Child Assessments
Provide patient education for chronic and high-risk conditions including asthma, diabetes, hypertension, and cardiovascular health
Deliver compassionate, patient-centered nursing care in a collaborative primary care environment
Assess patient conditions and develop individualized care plans
Administer medications and treatments in accordance with physician orders and nursing standards
Monitor patient status and respond appropriately to changes in condition
Accurately document patient information using electronic medical record systems such as AVA
Follow up with patients after hospital admissions, emergency department visits, and urgent care encounters
Perform full-scope nursing procedures including:
IV therapy
Phlebotomy
EKGs
Vaccinations
Assistance with minor procedures
Provide RN travel medicine consultations when required
Maintain inventory and organization of clinical supplies and equipment
Serve as a clinical resource and leader for team members including MOAs, RNs, and laboratory staff.
Collaborate effectively within a fast-paced interdisciplinary healthcare team
Communicate effectively and compassionately with patients experiencing distress or complex care needs
Participate in continuous learning, and innovation in primary and complex care
Ensure compliance with healthcare regulations, infection prevention standards, and clinic policies
Willingness to work occasional evening clinic hours and half-day Saturdays if required.
Maintain all required Alberta nursing licenses and certifications
Qualifications & Requirements
Current Registered Nurse (RN) practice permit in Alberta (CARNA/CRNA)
Previous experience in primary care, acute care, hospital, pediatrics, seniors care, or community health preferred
Strong understanding of patient-centered and team-based care models
Experience using electronic medical records systems; AVA experience is considered an asset
Experience in chronic disease management, pain management, dementia care, pediatrics, or case management is an asset
Laboratory experience including phlebotomy is an asset.
Excellent communication, organizational, and critical thinking skills
