Job Overview
We are seeking a dedicated, compassionate, and client-centered Registered Social Worker to join LumenPath Psychotherapy and Social Service work. The ideal candidate will play a vital role in providing psychotherapy, comprehensive social services, case assessment, and ongoing case management to individuals, children, adolescents, and families across clinical and community settings. The Registered Social Worker will collaborate closely with healthcare professionals, families, schools, and community agencies to ensure coordinated and client-focused support.
Duties
- Conduct comprehensive psychosocial, emotional, behavioural, and environmental assessments for clients, including children, adolescents, adults, and families.
- Provide individual psychotherapy and counselling using evidence-based modalities such as CBT, DBT-informed approaches, trauma-informed therapy, mindfulness-based interventions, solution-focused therapy, and supportive counselling.
- Develop and implement individualized treatment and care plans tailored to client needs and therapeutic goals.
- Provide crisis intervention, emotional support, coping strategies, and psychoeducation to clients and families.
- Support case management and resource coordination, including referrals to community services, educational supports, healthcare providers, and specialized programs.
- Assist with discharge planning and continuity of care by connecting clients with appropriate outpatient, community, or educational resources.
- Collaborate within multidisciplinary teams while utilizing knowledge of healthcare systems, mental health practices, and clinical documentation standards.
- Maintain accurate and timely documentation, including assessments, treatment plans, progress notes, and clinical reports in compliance with professional and regulatory standards.
- Support clients from diverse cultural and social backgrounds through culturally competent, trauma-informed, and inclusive care.
- Participate in program development, quality improvement initiatives, research activities, or data collection related to mental health and social work outcomes.
- Provide virtual and/or in-person psychotherapy services as required.
Skills & Qualifications
- Registered and in good standing with the Ontario College of Social Workers and Social Service Workers.
- MSW preferred.
- Professional Liability Insurance required.
- Strong interest in psychotherapy, mental health, and client-centered care.
- Experience providing counselling, psychotherapy, case management, practicum placements, or community mental health support is considered an asset.
- Training or familiarity with CBT, trauma-informed care, DBT-informed approaches, mindfulness-based interventions, or related therapeutic modalities is an asset.
- Strong communication, interpersonal, and organizational skills.
- Ability to work collaboratively within multidisciplinary teams.
- Compassionate, empathetic, professional, and eager-to-learn approach to care.
- Comfortable working in a supportive and growth-oriented environment with ongoing learning opportunities.
