Youth Wellness Facilitator (Youth Wellness Hub)
Location: Bancroft, ON (with outreach across NHCS sites and community)
Job Type: Full-Time, Permanent
Hours: 35 hours per week (including evenings and weekends as required)
About the Role North Hastings Children’s Services (NHCS) is seeking a passionate and youth-focused Youth Wellness Facilitator to join our Youth Wellness Hub team. As a core member of the Youth Wellness Team, you are often the first point of contact for youth, families, and community partners. You will help create a welcoming, inclusive, and youth-friendly environment while supporting young people to access the right services at the right time. This role plays a key part in intake, system navigation, and coordination, helping youth move seamlessly between skills & well-being, community supports, and clinical services within an integrated care model.
What You’ll Do
- Youth Engagement & Hub Environment
- Create a safe, welcoming, inclusive, and culturally responsive environment
- Greet youth and families and provide orientation to the Hub
- Build relationships with youth through one-on-one and group engagement Intake, Navigation & Coordination
- Lead the intake process using YWHO systems and screening tools
- Support youth in navigating services and transitions across care pathways
- Provide support in-person, virtually, and in the community Programming & Operations
- Plan and support drop-in programs, groups, and youth engagement activities Coordinate with partners for scheduling, space, and logistics
- Support low-barrier, strengths-based wellness programming Collaboration & Community Engagement
- Participate in team meetings, care planning, and case discussions
- Work closely with interdisciplinary partners to support seamless care Represent the Hub at community events and outreach initiatives Documentation & Continuous Improvement
- Maintain accurate documentation, case notes, and data tracking
- Support measurement-based care and evaluation activities
- Participate in quality improvement and provincial Communities of Practice
Who You Are
- Diploma or degree in social services or a related field (or equivalent experience)
- Minimum 1 year experience working with youth (ages 12–25)
- Knowledge of youth mental health, community services, and system navigation
- Committed to trauma-informed, anti-oppressive, harm reduction, and culturally safe practice
- Comfortable working in a fast-paced, team-based environment
Compensation & Benefits
- Wage: In accordance with the NHCS Salary Grid o Starting at $27.23 – $31.68/hour, depending on qualifications
- Group Benefits: Eligible after 1 month (as per plan terms)
- Statutory Holidays: As per ESA + additional NHCS paid holiday
Working Conditions
- Combination of on-site work, outreach, and travel within the community
- Evening and weekend work required
- Active, youth-facing role (program setup, movement, engagement)
