Summary:
The Registered Nurse (RN) at the Central Brampton Family Health Team (“CBFHT”) provides comprehensive primary care within an inter-professional, team-based model, collaborating closely with physicians, healthcare providers, and patients. As an integral team member, the RN delivers care in accordance with organizational policies, professional standards, and the regulations of the College of Nurses of Ontario, performing routine nursing duties while ensuring the clinical needs of patients and physicians are effectively met through coordinated, patient-centered care.
Accountability:
The Registered Nurse (RN) reports to, and is responsible to, the Executive Director. All clinical decision-making outside the scope of the Registered Nurse must be done in consultation of the CBFHT Lead Physician and/or physicians of CBFHT and/or medical directives.
Authority:
Does not direct the activities of staff or a function without direction of CBFHT decision makers. The RN must comply with the nursing practice standards used by the College of Nurses of Ontario.
Duties and Responsibilities:
- Patient Assessment & Care: Assess and monitor patients’ physical, emotional, and psychosocial health; interpret health records and diagnostic results; provide nursing care within full scope of practice, including immunizations, injections, venipuncture, spirometry, INR management, and cancer screening; participate in chronic disease management and prevention programs.
- Treatment & Management: Administer medications and treatments as prescribed; determine appropriate care, equipment, and services; ensure infection control, safety, and adherence to professional standards.
- Patient Education & Advocacy: Educate patients and families on health conditions, treatment, and preventive care; promote self-management and access to community resources; develop and deliver educational materials and programs.
- Care Coordination & Collaboration: Collaborate with physicians, healthcare providers, and community services to plan, implement, and evaluate patient care; contribute to problem-solving and improving patient outcomes.
- Administration & Operations: Coordinate patient flow, triage, appointments, and follow-ups; document care accurately; manage clinic equipment, inventory, and supplies; handle specimens safely; prepare required reports.
- Professional Practice & Team Support: Practice in accordance with professional standards and organizational policies; work independently and collaboratively; assist with onboarding, mentoring, and quality improvement initiatives.
The RN will participate in our Chronic Disease Management and Health Prevention Programs. An expertise in CDM programs is required.
Education/Experience:
- Member in good standing with the College of Nurses of Ontario
- Baccalaureate degree in Nursing (B.Sc.N)
- Preference given to those with at least two years of nursing experience in a Primary Care setting
- Practical experience in Palliative, Geriatric, Diabetes and Lung health program team collaboration (Spirometry experience an asset)
- Current CPR
- Must have own vehicle and valid driver’s license
- Knowledge of and ability to use electronic medical records
- Strong organizational, interpersonal and communication skills
- Ability to work independently and cooperatively in a busy multi-disciplinary team
- Have the ability to act responsibly, accountable, excellent problem solving/critical and thinking skills.
Skills and Abilities:
- Demonstrated ability in utilizing the nursing process in planning, implementing and evaluating patient care;
- Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction;
- Leadership, organization, research, evaluation, time management, communication and interpersonal skills;
- Demonstrated ability in problem solving using sound judgment and critical thinking;
- Strong organizational, interpersonal and communication skills;
- Ability to prioritize, manage time effectively and be flexible in a very active constantly changing work environment which includes being flexible to work daytime and evening hours;
- Proficiency in the use of computers and information technology is required, including the use of electronic medical records.
