Reports To: Community Support Supervisor
Location: Various community locations and individuals’ homes
Job Type: Full-time / Part-time
Salary Range: $23 - $25/hour
Position Summary:
The Community Support Worker provides individualized, person-centered support to individuals served by Connect Community Living (CCL). This includes helping with daily activities, encouraging independence, and promoting community participation. Regular updates on progress and challenges are shared with the Community Support Supervisor.
Key Responsibilities:
- Direct Support:
- Assist with personal care tasks such as grooming, cooking, and medication management while supporting daily living skills. Facilitate community engagement through activities and outings, ensuring support aligns with each individual’s Personalized Service Plan (PSP).
- Documentation and Reporting:
Maintain accurate records of activities, progress, and incidents. Submit timely reports to the Community Support Supervisor and ensure confidentiality is upheld.
Work closely with other support workers, Homeshare Providers, and the CCL management team to provide consistent care. Attend staff meetings and trainings to stay aligned with best practices.Follow health and safety protocols, assist individuals with medical appointments, and respond to emergencies. Report incidents to the supervisor as needed.Qualifications:
- Health and Safety:
- Certification in Community Support, Health Care, Social Work, or a related field.
- Experience working with individuals with disabilities or in community support settings is preferred.
- Strong communication and interpersonal skills, with the ability to manage challenging behaviors professionally.
- Organizational skills for documenting and reporting activities accurately.
- Flexibility to work varying shifts, including evenings, weekends, and holidays.
- Basic computer literacy for maintaining records and communication.
Driving and Vehicle Requirements:
A reliable vehicle, valid driver’s license, clean driving record, and insurance with a minimum of $2,000,000 third-party liability coverage are required.
Other Requirements:
- Valid First Aid/CPR certification.
- Clear criminal record check.
Work Environment:
This role involves travel to various community locations and individuals’ homes, with some physical tasks such as assisting with mobility.
