Job Description – Occupational Therapist (Long-Term Care)
Position Title: Occupational Therapist (OT)
Department: Rehabilitation Services
Reports To: Director of Care/Clinical Manager
Position Summary
The Occupational Therapist (OT) is responsible for assessing, planning, implementing, and evaluating occupational therapy services for residents in a long-term care (LTC) setting. The OT works collaboratively with residents, families, physicians, and the interdisciplinary team to maximize residents' independence, safety, mobility, and quality of life through therapeutic interventions and adaptive strategies.
Key Responsibilities
- Complete comprehensive occupational therapy assessments for residents upon admission and as required.
- Develop, implement, and evaluate individualized treatment plans based on residents' goals and functional abilities.
- Assess residents' ability to perform activities of daily living (ADLs) and recommend interventions to maximize independence.
- Recommend, prescribe, and train residents and staff in the safe use of adaptive equipment, mobility aids, splints, seating, and positioning devices.
- Conduct wheelchair, seating, and positioning assessments to promote comfort, skin integrity, and functional mobility.
- Develop fall prevention strategies and recommend environmental modifications to reduce safety risks.
- Provide education and support to residents, families, and staff regarding therapeutic techniques, equipment use, and safe care practices.
- Collaborate with physicians, nursing staff, physiotherapists, recreation therapists, dietitians, and other members of the interdisciplinary team to develop and maintain resident-centered care plans.
- Monitor residents' progress and modify treatment plans as needed.
- Maintain accurate, timely, and confidential clinical documentation in accordance with professional standards and facility policies.
- Participate in interdisciplinary care conferences, quality improvement initiatives, and discharge planning where applicable.
- Ensure compliance with infection prevention and control practices, occupational health and safety regulations, and applicable legislation.
- Maintain professional competency through continuing education and adherence to the standards of the provincial regulatory college.
Qualifications
- Bachelor's or Master's degree in Occupational Therapy from an accredited program.
- Current registration and licensure in good standing with the applicable provincial regulatory body.
- Experience working in long-term care, geriatrics, rehabilitation, or a related setting is preferred.
- Knowledge of aging, dementia, cognitive impairment, falls prevention, seating and positioning, and adaptive equipment.
- Excellent assessment, communication, documentation, and organizational skills.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Current CPR certification and other certifications as required by the employer.
