Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. This role offers an excellent opportunity to provide high-quality patient care across diverse settings, including pediatrics, senior care, and home health. The ideal candidate will have a strong background in patient assessment, medication management, and medical documentation, with experience working with various patient populations. The Nurse Practitioner will collaborate with physicians and healthcare staff to deliver comprehensive care tailored to individual patient needs, ensuring safety, comfort, and optimal health outcomes.
Responsibilities
- Provide comprehensive, patient-centred primary care across the lifespan, including assessment, diagnosis, treatment, and ongoing management of acute, chronic, and complex health conditions.
- Develop and implement individualized care plans in collaboration with patients, families, and the interprofessional team to optimize health outcomes.
- Deliver preventive care and health promotion services, including routine screening, immunizations, well-woman care, well-baby and well-child care, prenatal care, and patient education.
- Conduct medication reviews, monitor investigations, and support chronic disease management, self-management, and health literacy.
- Assess and manage urgent and episodic health concerns, providing timely intervention, triage, referral, and follow-up within the Nurse Practitioner scope of practice.
- Coordinate care for patients with complex medical and social needs by collaborating with physicians, specialists, allied health professionals, community agencies, and other healthcare providers to ensure seamless transitions and continuity of care.
- Facilitate access to community resources and support services that address health and social determinants of health.
- Maintain accurate, timely, and comprehensive clinical documentation in accordance with professional standards and organizational policies.
- Participate in quality improvement initiatives, program evaluation, clinical protocol development, and evidence-informed practice to enhance patient care and organizational performance.
- Comply with all healthcare regulations, safety protocols, and privacy laws
Qualifications and Skills
- Current registration in good standing with the College of Nurses of Ontario as a Nurse Practitioner (Extended Class).
- Master’s degree in Nursing or completion of a recognized Nurse Practitioner education program.
- Current Basic Life Support (BLS/CPR) certification.
- Demonstrated commitment to providing equitable, trauma-informed, culturally safe, and patient-centred primary care.
- Strong clinical assessment, diagnostic, prescribing, and independent decision-making skills within the Nurse Practitioner scope of practice.
- Excellent interpersonal, communication, and collaboration skills, with the ability to build therapeutic relationships with patients from diverse backgrounds.
- Proficiency in electronic medical records and other healthcare technologies; experience with primary care EMR systems is considered an asset.
- Commitment to continuous professional development, quality improvement, and interprofessional team-based care.
