Community Living Burlington is a social-profit charitable organization serving the communities across the Halton region since 1955. Our mission is to enrich the quality of life of people with a developmental disability and promote their full and meaningful inclusion in the community. We are looking for a highly motivated self-directed person who will act as liaison and a community representative on behalf of Community Living Burlington in a Live in Coordinator (LIC) position. This position will primarily work directly on the floor to ensure that the individual’s programs and goals are being met.The Live in Coordinator is an essential role in the agency. This person will support four (4) adults with developmental disabilities in the home they share. The LIC acts as a supportive housemate, providing personal care and meal preparation along with coordinating the daily schedule of the residents.
This is a shared position with a counterpart – with two other coordinators completing the week to ensure the residents are supported at all times.
Under the general supervision of the Supported living manager, the Live-In Coordinator provides a wide range of services to people with developmental disabilities who live and thrive in a 24-hour support home. The incumbent in this position is required to enable, support and maintain a positive environment by working together with the families towards a common goal, providing support and maintaining open communication. The Live-In Coordinator will be working with care plans, personal support plans, and all relative documentation to ensure these adults receive the best care possible while enriching their lives.
The successful candidate will live in a privately funded group home (staffed by Community Living Burlington) during the time above. The home is a well-maintained house in a family friendly neighborhood. The successful candidate will have use of his or her own suite/bathroom, with the expectation of maintaining the space with respect to sharing the space with their counterpart.

You demonstrate an understanding of leadership principles and solid track record of success in developing others. You must be able to work flexibly within a team structure.
Key Requirements
- University or College Degree in the Human Services Field, such as Developmental Services Worker, Social Services Worker, Child and Youth Worker, Early Childhood Educator, Bachelor of Arts or Science in the Human Services. We do not consider Personal Support Worker Diplomas
- Demonstrated knowledge of health and safety requirements for 24-hour support home facilities
- The ability to act as a liaison with community agencies and partners in regard to creating new and existing partnerships and opportunities
- Previous experience working with people who have a developmental disability is an asset.
- Ability to work independently and as part of a team
- Excellent problem solving skills that can be applied in multiple situations
- Skills to work collaboratively in a team, promote effective communication and have strong skills in problem solving and conflict resolution.
- Must provide a police clearance with vulnerable sector search
- Driver’s License- level G and a reliable vehicle
- Proficient in Microsoft Office Suite

- To maintain a welcoming home environment and promote a harmonious and respectful family atmosphere among the people who live in the home.
- Overall care and supervision of people who reside in the home.
- To provide personal care to four (4) adults with a developmental disability
- To ensure the core values of Community Living Burlington are being met
- To adhere to all policies, guidelines and regulations as directed by the agency for the security and well-being of those supported
- To work as a team to ensure excellent care of the residents.
- Report directly to the Senior Manager of Community & Resource Development
- Provide training and direction to full-time and part-time support workers
- Quality Assurance Measures (QAM) in Accordance to the Ministry of Community and Social Services
- Ensuring the safety and security of the people who reside in the home
- Supervising meal times and involving everyone in meal preparation and other activities within their home
- Ensuring that all identified support needs of everyone are met as may be identified by Person Centered Plans, families and CLB
- Ensuring the maintenance, repair and upkeep of the house and CLB vehicle(s)
- Such other duties and responsibilities related to the position of Live-In Coordinator as you may be required to perform or as may be assigned to you from time to time.
- You will be responsible for overseeing a monthly budget for the operation of the House, including the purchase of groceries and other household items as may be necessary for the performance of your duties and responsibilities under this Agreement.
