Position Overview
Case management involves coordinating all medical, paramedical, and psychological care to meet the needs and preferences of individuals living with chronic and/or complex conditions, including those experiencing severe and persistent concurrent disorders (mental health and substance use issues).
The case manager is primarily responsible for the following: multidisciplinary care coordination; identifying clients who require an integrated care approach, particularly individuals presenting with severe and persistent concurrent disorders; patient advocacy; liaison with external agencies; sharing the care plan with the treatment team; and engaging family members and loved ones.
As part of their duties, the case manager may be required to work across different sites based on organizational needs.

What we are looking for
- Bachelor's in social work.
- Member in good standing with a professional college in Ontario.
- Minimum of three (3) years of experience in mental health and addictions/gambling and concurrent disorders.
- Be able to correctly assess a situation based on a thorough analysis of the needs, resources and wishes of the patient and his or her family and friends;
- Be able to plan and coordinate care;
- Be able to complete the liaison and service transition;
- Have a thorough knowledge of the social context in a defined area;
- Have a thorough knowledge of the functions and roles of the different care providers;
- Have a good understanding of the limitations of the system's services;
- Be able to build a quality relationship with the patient, family caregivers and caregivers;
- Be able to empower patients
- Ability to work in a multidisciplinary environment
- Possess a working knowledge of both official languages.
