Position Summary
Guided by the principles of primary health care and Indigenous ways of knowing, the Primary Care Nurse provides holistic nursing care to individuals, families, and the wider community. This role supports wellness across the lifespan by integrating Western nursing practice with Indigenous cultural teachings, emphasizing prevention, health promotion, chronic disease management, and compassionate clinical care.
The Primary Care Nurse provides direct medical services that include primary patient care and treatment, health promotion and education, disease prevention, and referrals to traditional health and wellness resources. Care is delivered in a manner that honours physical, emotional, mental, and spiritual wellbeing.
The Primary Care Nurse functions as a generalist, with a strong focus on community health, preventive care, and health education to improve population health outcomes. The role works closely with families and community members to address social and structural determinants of health, while fostering trust, dignity, and cultural safety.
As a member of a collaborative, multidisciplinary primary care team, the Primary Care Nurse actively contributes to a trauma‑informed, racism‑free, and culturally safe environment, demonstrating cultural humility and respect for Indigenous values, teachings, and protocols.
Key Responsibilities and Duties
Culturally Safe Patient Assessment & Care
· Conduct holistic health assessments that honour Indigenous worldviews and lived experience.
- Monitor and record vital signs; observe and assess physical, emotional, mental, and spiritual wellbeing.
- Provide compassionate, strengths‑based nursing care and emotional support to clients and families.
Treatment Administration
· Administer medications and immunizations in accordance with nursing standards and community protocols.
- Perform wound care, phlebotomy, IV therapy, and other nursing interventions within scope of practice.
- Apply harm‑reduction and preventive approaches grounded in respect and non‑judgment.
Care Coordination & Collaboration
· Work in partnership with Nurse Practitioners, Physicians, specialists, and allied health providers to develop, implement, and update client‑centred care plans.
- Support continuity of care by linking clients with community, clinical, and traditional wellness resources.
- Participate in team‑based planning and service coordination.
Health Promotion, Education & Community Engagement
· Provide culturally relevant health education to individuals, families, and groups on wellness, chronic disease management, and illness prevention.
- Support community clinics and programs (e.g., diabetes, prenatal, men’s and women’s health, immunization).
- Engage in home and community visits to support outreach, prevention, and early intervention.
Documentation & Accountability
· Maintain accurate, timely, and confidential documentation using Electronic Medical Records (Accuro and Mustimuhw).
- Record assessments, care provided, and outcomes in accordance with professional, legal, and organizational standards.
- Support accreditation, quality improvement, and reporting requirements as assigned.
Emergency & Acute Response
· Respond promptly to medical emergencies during work hours.
- Implement appropriate acute care interventions within scope of practice while ensuring client safety and dignity.
Professional Practice & Expectations
· Practice in accordance with the standards of the College of Registered Nurses of Manitoba.
· Provides nursing care to clients with highly complex needs, including comprehensive assessment, care coordination, and ongoing monitoring.
· Demonstrates the ability to use non‑violent de‑escalation techniques to manage escalated behaviours while maintaining client dignity and safety.
· The Primary Care Nurse is required to work alone at times and must be capable of managing patient care independently.
- Demonstrate ongoing commitment to cultural safety, humility, and trauma‑ and violence‑informed care.
- Build and maintain respectful relationships with community members, leadership, and partner agencies.
Experience
- A combination of relevant nursing experience will be considered.
- Minimum of two (2) years of recent, related experience in primary care, public health, community health, and/or chronic disease management.
- Minimum of two (2) years experience providing health services to First Nations and Indigenous communities, including working within community‑based and culturally grounded care models.
- Experience or training in mental health support, crisis response, and suicide prevention (e.g., ASIST) is an asset.
- Demonstrated training and practice in anti‑Indigenous racism response, cultural safety, and cultural humility.
- Experience integrating holistic Indigenous approaches to health and wellness with Western clinical nursing practice, using strengths‑based and trauma‑informed care approaches.
Education
- Graduate of an approved Registered Nursing education program; Bachelor of Nursing (BN) or Registered Nurse (Extended Practice / RNAP) required.
- ISC Immunization Certification preferred.
- Certification or coursework in Diabetes Education and/or Chronic Disease Management preferred.
Certification / Licensure
- Current registration in good standing with the College of Registered Nurses of Manitoba (CRNM).
- Active participation in professional nursing associations (e.g., Association of Regulated Nurses of Manitoba – ARNM) is an asset.
- Current CPR certification in accordance with employer policy.
- Valid Manitoba Class 5 Driver’s Licence and ability to travel within the community as required.
Accountability
Reports to the Primary Care Clinic Manager and works in close collaboration with Physicians, Nurse Practitioners, and other Clinical Staff to support integrated, team‑based primary care services.
