Person-and family-centered care (PFCC) is an approach to care that recognizes the importance of family in a patient’s life and the importance of their active involvement in planning and making decisions on health care, services and treatment, and health system reform. This approach to care provides respectful, compassionate, culturally safe, and responsive care that meets the needs, values, beliefs, and preferences of the patient, their family, and others identified as significant to their life from diverse backgrounds and settings.
DUTIES AND RESPONSIBILITIES
- Execute the social and wellness care plan under the direct supervision of a Licensed Social Worker.
- Provide in-person and virtual support to patients and families that support the activities in the care plan including wellness checks, food & housing securities as well as social community support connections.
- Engage with various and many Community Support services to follow up on referrals by the SW to ensure referrals are being actioned and there is an expected date.
- Arrange and manage transportation services for patients to community appointments and at times accompany them to and from the appointment.
- Maintain and manage a caseload efficiently, ensuring priority code timeframes are met.
- Utilize a client-centered approach to identify, prioritize, and monitor potential psychosocial problems and goals.
- Provide education in person or educational resources (online, in person, paper) that best meet the patients need to support specific patient conditions/diseases or social concerns.
- Advise clients/caregivers on the availability of community resources and facilitate referrals when appropriate.
- Determine when further social work intervention is no longer necessary and assist clients in planning for discharge.
- Comply with continuous quality improvement initiatives.
- Adhere to Bayshore Policies and Procedures.
- Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
- Participate in proactive Health & Safety activities.
- Maintain confidentiality of client and corporate information.
- Complete other tasks as requested.
- Participate in ongoing internal and/or external continuing education activities
- Adhere to Bayshore Policies and Procedures.
- Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel
- Document and report all tasks/information according to Bayshore policy.
- Maintain treatment records in accordance with the Standards of Practice of the Ontario College of Certified Social Workers and Social Service Workers.
- Demonstrate agility in decision-making and think critically to respond to a rapidly changing environment.
- Maintain positive and collaborative working relationships within the interdisciplinary health care team.
- Demonstrate the ability to be self-directed and thrive in a dynamic environment.
- Able to remain calm under pressure and role model composure.
Qualifications:
- Possess a Diploma in Social Service Work.
- Be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers.
- Provide a police clearance that meets the Bayshore Home Health policy.
- Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities.
- Minimum of two years of community-based therapy experience (preferred).
- Be able to drive, hold a current, valid driver’s license, and have access to a vehicle in good operating condition (preferred).
