POSITION SUMMARY:
The Registered Nurse at the Regional Centre (OSMH) of the RKCP SM is responsible for comprehensive, co-ordinated and individualized care of patients with end stage renal disease and acute kidney injury requiring hemodialysis treatments in accordance with the mission, vision and goals of the program. The Registered Nurse is an active member of the multidisciplinary care team which provides care and support to patients and families across the Regional Program. The hemodialysis RN provides hemodialysis and renal care to both inpatients and outpatients of the program. This includes acute care treatments across multiple sites of the program. Travel is required.
PRIMARY RESPONSIBILITIES:
General:
- Meeting the College of Nurses of Ontario Standards of Nursing Practice, and functions within the policies and procedures of the Patient
Care Services.
- Organizing and implementing a plan of nursing care to meet the needs of each assigned patient.
- Coordinating the patient care activities and providing effective supervision to others to whom he/she delegates activities.
- Effectively utilizing human and material resources.
- Participating in the orientation of new staff members to the nursing unit.
- Attending educational programs and in-services to enhance his/her knowledge, and participating in the planning and implementing of
same.
- Assessing his/her own professional development needs, and sets goals to meet same.
- Assisting in maintaining the established standards of care through individual performance, participation in hospital committees, and
assisting with workload analysis, Audits etc.
- Cooperating with clinical instructors to provide an educational environment for nursing students. Interpreting and demonstrating
skilled nursing care to students. Participates in preceptor program.
- Maintaining an awareness of current developments in nursing practice.
- Performing Charge Nurse role when assigned.
- Taking responsibility for the safety and welfare of patients.
- Functioning in accordance with established hospital emergency procedures
- Promoting inter and intradepartmental cooperation by effective communication.
- Participating as a member of the multidisciplinary team and promoting effective liaisons between patient, family and physician, and
other health team members and community agencies. Encourages referrals to appropriate community resources.
- Demonstrating knowledge of and maintains competence in accordance with RHPA..
- Participating in assessing health needs and promoting health education and maintenance through patient and family teaching
programs.
Specific:
- Carrying out hemodialysis treatments, including accessing of AV-fistula, grafts and central vein dialysis catheters
- Participating as an active member of the multidisciplinary team of the renal program
- Acting as a role model in clinical excellence in representing the regional centre of the RKCP SM
- Acting as a clinical resource person to members of other nursing departments and other health care professionals on the management
of patients with ESRD across RKCP SM
- Carrying a primary patient case load
Performs other duties as may be assigned by the Program Manager. Potential to be utilized in other patient care areas.
QUALIFICATIONS:
Education:
- Current certificate of competence to practice as a Registered Nurse in Ontario is required.
- BScN preferred
- CNeph (c) preferred
Experience:
- Minimum one (2) year recent hemodialysis experience required.
