Summary:
- The Planning Specialist has the lead responsibility for facilitating the development and implementation of SASH’s outcome-based Service Delivery System for individuals receiving support and services. The planning specialist leads this process for all Outreach/Contract and Magrath Day Program individuals and acts as a resource to the supervisors and coordinators of all other (1010) home living supports to ensure consistency of supports throughout the agency. The Planning Specialist provides orientation, training, information/education to individuals, their support networks, and staff as detailed and outlined in the SASH outcome-based service delivery system. The Planning Specialist works closely with the entire support team especially with the Behavioral Specialist in creating, orientating, training, administering, evaluating, and interpreting, environmental interventions, assistive technology plans according to the SASH Individual rights manual. He or she will guide individuals towards the development of healthy behaviors and well-being. Other tasks may be assigned, as necessary.
Qualifications:
- Diploma in a related behavioral discipline (Preferred).
- Support Employment Certificate (Completed in first 3 months)
- Person Centered Planning Certificate (Completed in first 3 months)
- Experience working with individuals with developmental disabilities.
- A clear criminal record check including the vulnerable sector is required.
- Well-developed observation and interpretive skills to support judgments about issue resolution and required actions, this includes the ability to identify observable changes in behavior and attitude and provide advice and support to others as required.
- Effective time and priority management skills and the ability to balance competing demands/priorities and deal with competing opinions.
- Emergency First Aid/CPR, APRP, PBS, SIVA and Medication Administration certifications are required or must be obtained within 90 days, training provided through agency or provide your current certification.
- Strong computer skills & Proficient in Microsoft Office.
- Reliable vehicle, class 5 driver’s license, clean drivers abstract, and $2,000,000 liability required.
- Excellent verbal & written communication skills with the ability to create and train on complex behavioral support plans.
Salary: $22.89 - $26.00 and is dependent on education and experience.
