Position Overview
Our Happy Place Supportive Services (OHP) is a community-based day program providing enrichment, recreation, and meaningful social experiences for adults with neurological and/or physical diversity.
We are currently seeking compassionate, reliable Social Service Workers (SSWs) to join our team. As an SSW at OHP, you will play a vital role in supporting participants with daily living activities while promoting dignity, independence, inclusion, and overall well-being in a warm, respectful environment.
You will work collaboratively with Recreation Programmers and Program Directors to support participants both on-site and in the community.
Requirements
- Social Service Worker (SSW) Diploma from a recognized college
- Current First Aid and CPR certification
- Current Vulnerable Sector Police Record Check (or willingness to obtain prior to start date)
- Reliable transportation to and from the program location
- A strong commitment to person-centred, respectful care practices
- Excellent communication and interpersonal skills
- Patience, flexibility, adaptability, and a team-oriented mindset
- Ability to maintain professionalism, boundaries, and confidentiality at all times
Experience supporting individuals with neurological and/or physical diversity is considered an asset.
Key Responsibilities
- Provide respectful, person-centred support to adults with neurological and/or physical diversity
- Assist participants with activities of daily living, including personal care, hygiene, toileting, feeding, and mobility as required
- Support participants during recreational activities, group programs, and community outings
- Encourage independence, choice-making, and self-expression while ensuring safety and dignity
- Assist with transfers, positioning, and mobility supports as needed
- Monitor participant well-being and communicate concerns to the program team
- Support meal and snack preparation and assist participants during mealtimes
- Maintain accurate documentation, participant notes, and reports as required
- Uphold infection prevention and control practices
- Help maintain a clean, safe, and organized program environment
- Participate in ongoing training and professional development
Additional Responsibilities May Include
- Implementing behaviour support strategies as directed by the program team
- Providing changing and incontinence care respectfully and discreetly
- Supporting participants during transitions
- Participating in team meetings and contributing to program development
- Other related duties as reasonably assigned by the Program Director
