Who we are
Transitional Support Services provides trauma-informed and relationship-based care to children and youth through therapeutic group homes and outreach services. The young people we work with have often experienced trauma and loss and may present with a wide range of adaptive behaviours.
Your role
The Post Care Case Worker, under the direct supervision of the Post Care Unit Supervisor, delivers comprehensive case management services to adult youth aged 19-26 eligible for post-care support through Family and Children Services. The primary duty of this role is to aid adult youth, formerly under the Director’s care in transitioning successfully towards independence while adhering to the structured framework of Family & Children Services and current legislation.

Essential qualifications

- Experience in collaborating with clients to develop individual case plans with short-term and long-term goals
- Experience working with adult youth 19-26 years who have transitioned from Director’s care
- Experience working with First Nation Government and agencies to access programs and services and promote cultural connections

- Strong team leader with excellent interpersonal skills
- Strong writing skills and oral communication skills
- Experience conducting intakes on adult youth
- Experience with internal and external service providers

RCMP security clearance with vulnerable sector, valid Yukon Class 5 driver’s license, First Aid and CPR
