Position Summary
SAMH is seeking a highly organized, compassionate, and detail-oriented Registered Practical Nurse (RPN) to join our multidisciplinary team. The successful candidate will play a critical role in ensuring the safe administration of medications, accurate clinical documentation, effective communication with healthcare providers, and compliance with professional nursing standards.
This position requires a nurse who demonstrates exceptional accountability, strong critical thinking skills, and a commitment to maintaining the highest standards of client safety and care.
Key Responsibilities
Medication Management
- Safely administer medications in accordance with physician orders, College of Nurses of Ontario (CNO) standards, and SAMH policies.
- Maintain accurate and complete Medication Administration Records (MARs).
- Verify physician orders and ensure all medication changes are implemented promptly and correctly.
- Conduct medication audits and reconciliation as required.
- Monitor, document, and report medication effectiveness, side effects, and adverse reactions.
- Ensure controlled substances are accurately documented and accounted for.
Clinical Documentation
- Complete all nursing documentation accurately, thoroughly, and within required timelines.
- Maintain detailed client health records, assessments, progress notes, and incident reports.
- Review documentation regularly to ensure completeness and compliance.
- Identify and correct documentation discrepancies immediately.
Client Care
- Assess clients' physical and mental health status.
- Monitor withdrawal symptoms and recovery progress.
- Participate in treatment planning and case management.
- Provide health education and support to clients and families.
- Respond appropriately to medical emergencies and crisis situations.
Collaboration & Communication
- Communicate effectively with physicians, psychiatrists, pharmacists, management, and other healthcare professionals.
- Participate in team meetings, case conferences, and quality improvement initiatives.
- Escalate concerns promptly when client safety may be compromised.
Quality Assurance & Compliance
- Adhere to all CNO standards, Accreditation Canada requirements, privacy legislation, and SAMH policies.
- Participate in audits, accreditation activities, and continuous quality improvement initiatives.
- Demonstrate accountability for clinical practice and professional responsibilities.
Required Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO).
- Current CPR and First Aid certification.
- Minimum 2 years of nursing experience preferred.
- Experience in addictions, mental health, community health, residential treatment, or related settings is considered an asset.
- Valid driver's licence and reliable transportation.

Job Types: Full-time, Permanent
Pay: $28.00-$32.00 per hour
