Job Summary
The Activity Coordinator plays a vital role in enhancing the quality of life for residents in our facility. This position is responsible for planning, organizing, and implementing engaging activities that cater to the diverse interests and abilities of residents, including those with dementia and other cognitive impairments. The ideal candidate will possess strong communication skills and a passion for caregiving, ensuring that all activities foster social interaction and promote physical, mental, and emotional well-being.
Activity Duties:
- Develop and implement a comprehensive activity program tailored to the needs and preferences of residents.
- Organize individual and group activities that promote socialization, creativity, and physical fitness.
- Include residents, families, and House Teams in the planning of House calendars.
- Create opportunities to celebrate special occasions and life events.
- Assist resident to and from activities.
- Foster and maintain community-linked partnerships.
- Provide individualized programs and visitations regularly.
- Communicate with residents, House Teams, families, volunteers, and community partners regarding programs and events.
- Assist with daily dining experience.
Assessment of Residents:
- Function as a member of the Interdisciplinary Care Team and attend Resident Care Review meetings as scheduled and complete Resident Care Review Assessment Forms prior to the meeting.
- Assess resident’s leisure interests and lifestyle choices through consultation with residents and families.
- Under the direction of the Recreation Therapy Manager, document on resident progress notes and care plans as required and input information in the Leisure Lifestyle section of the electronic Care Plan.
- Establish an activity program to promote resident’s involvement and choice by developing an individualized program plan for each resident.
Administrative Duties:
- Work with and supervise volunteers and students assigned to the department.
- Input resident’s participation daily through Momentum Program (LTCF).
- Attend conferences and/or workshops related to the field of Therapeutic Recreation as required.
- Participate in fundraising for Department when required.
Qualifications:
- University Degree/Diploma in Therapeutic Recreation or post-secondary completion of certified course in Recreation Programming plus coursework in gerontology/psychology is preferred.
- Ability to communicate and relate to residents challenged by age or disability.
- Must be capable of working independently, with good organizational and planning skills.
- Class 4 driver's license is an asset
- Proficient in Microsoft Office products
- Must be able to prepare reports as required.
- Team player - ability to work well with others and function as a Team member.
- Good previous work and attendance record.
- Physically able to perform all assigned duties.
- Must provide a Criminal Record Check/Vulnerable Persons Check.
- Must pass a Department of Social Development Record Check
- Must pass pre-employment medical.
This role requires a dedicated individual who is enthusiastic about creating meaningful experiences for residents while promoting their overall well-being.
