Family Care Coordinator — Oshawa, ON
Part‑time / flexible (potential to grow) Independant Contract Position
We are a family in Oshawa seeking a reliable, organized Family Care Coordinator to coordinate and support a small care team for our adult son who lives with complex needs. This role acts like an executive assistant to the family: you will be the operational and communication hub that helps the support team run smoothly and consistently.
This is not a direct care, nursing, or PSW role.
What you’ll do:
- Coordinate and schedule staff shifts, appointments, and team meetings; manage scheduling changes and shift coverage.
- Track team hours and maintain an up‑to‑date roster.
- Monitor and track adherence to the plan-of-care.
- Maintain and organize documentation, digital records, and tracking sheets; prepare simple reports or summaries for the family.
- Serve as the central communication point for the care team: filter general inquiries, flag critical items for the family, and follow up on outstanding issues.
- Organize team check‑ins, prepare agendas, and take notes.
- Research community programs, volunteer roles, and connection opportunities; share options and help with basic logistics.
- Research new home activities and plan for developing new interests;
- Support onboarding of new team members: share role expectations, prepare and update onboarding documents, and explain routines and communication norms.
- Light home upkeeping: basic household tasks to maintain a tidy, safe environment (e.g., tidying common areas, organizing supplies, coordinating minor household maintenance).
- Provide non-clinical supervision to cover staff breaks and ensure safety and routine adherence.
- Accompany support workers on outings as observational support to monitor routines and communication, and report back to the family.
Qualifications & Skills
- Minimum 2 years in administration, executive assistance, project coordination, or case management.
- Experience in disability, healthcare, or social services is an asset but not required.
- Strong proficiency with Google Workspace and/or Microsoft 365 (calendars, docs, spreadsheets).
- Excellent organization, attention to detail, and comfort with digital records.
- Resourceful, proactive, and able to anticipate administrative/care-team needs.
- Exceptional written and oral communication; collaborative, respectful, and non‑intrusive in a home environment.
- Calm, professional approach to juggling multiple demands.
- Absolute commitment to family privacy, confidentiality, and professional boundaries.
- Genuine empathy and respect for people with disabilities.
- Valid drivers licence and reliable vehicle; comfortable driving to appointments and community activities.
This role is not responsible for personal care tasks, medical procedures, or direct nursing/PSW duties.
Availability
- Part‑time with flexible hours. Must be available for occasional weekend and evening shifts as needed.
- Must be available to travel locally by vehicle for outings and occational community activities.
