Role Overview
As we continue to expand, we are building a small, reliable team of experienced professionals who can conduct client assessments and intake visits on an as-needed basis. This position is designed for individuals seeking a flexible role, with the potential to grow into a more consistent Care Coordinator or leadership position.
The coordinator will work closely with the Care Director to gather client information, assess care needs, and develop individualized care plans, ensuring a smooth and professional onboarding experience.
Key Responsibilities
Client Intake & Onboarding
- Conduct initial home/virtual assessments to evaluate client needs, safety, and personal preferences.
- Assist with onboarding new clients
- Develop, implement, and update personalized Care Plans based on client’s needs.
- Maintain detailed, accurate documentation of all assessments, visits, and care plan modifications.
Client & Family Communication
- Represent the service provider (Heartwell) professionally during initial client and family interactions
- Help set clear expectations regarding services and support
Qualifications
- Healthcare background required (PSW, RPN, RN, or related field)
- Valid Vulnerable Sector Check (or willingness to obtain)
- Valid G driver’s license with a clean driving record (preferred)
- Reliable vehicle with appropriate insurance
- Nursing background (RPN/RN) is an asset
- Additional healthcare-related diploma or degree is an asset
- Current CPR / First Aid certification is an asset
Essential Skills & Qualities
- 1–3+ years of experience in home care, client assessments, or care coordination
- Experience conducting client assessments, developing care plans, or participating in intake processes
- Experience working in a healthcare or medical environment
- Strong knowledge of medical terminology and healthcare documentation
- Experience working with PSWs and/or nurses
- Ability to handle sensitive situations with professionalism, empathy, and sound judgment
- Strong organizational skills and ability to work independently
- Professional communication skills (written and verbal) with clients, families, and care teams
- Detail-oriented with accurate documentation practices
- Strong networking and relationship-building skills
Travel Requirements
- Need based travel throughout your specific region
Compensation
- contract-based role
- Pay per assessment
