Job Overview
We are seeking a highly skilled and motivated Nurse Case Manager (NP or RN) to lead and coordinate our Health, Hormone, and Longevity programs within a physician-led, premium clinical setting.
We are well recognized leaders in the field of women's health, longevity and regenerative medicine and embarking on an exciting growth trajectory.
You will support the development and implementation of personalized patient care journeys focused on women’s health, hormone optimization therapy (HOT), metabolic performance, preventive diagnostics, and longevity medicine. You will integrate advanced clinical skills with structured patient education and high-touch care coordination for best in class medical care delivered with exceptional client experience.
This role is ideal for a confident, clinically strong nurse with a passion for women’s health, integrative medicine, longevity and regenerative science and performance optimization.
Core Responsibilities
Clinical Assessment & Program Implementation
- Conduct comprehensive patient assessments (vital signs, medical history, physiology review)
- Support hormone and health optimization programs
- Perform core clinical skills: IV therapy, IV insertion, and phlebotomy
- Administer IM and subcutaneous injections
- Perform advanced testing, women's health procedures, in clinic diagnostics (within scope)
- Assist with advanced procedures (medical, women's health, cosmetic)
- Maintain precise EMR documentation and care tracking
Case Management & Care Coordination
- Develop and implement individualized care plans aligned with longevity and performance goals
- Coordinate diagnostic testing and laboratory follow-up
- Monitor treatment progress and adjust plans in collaboration with the Medical Director
- Ensure seamless patient transitions between consultation, diagnostics, and follow-up care
- Provide structured follow-up to optimize adherence and outcomes
Patient Education & Engagement
- Educate patients on hormone therapy, metabolic health, and preventive care strategies
- Reinforce lifestyle, supplement, and therapeutic recommendations
- Support long-term retention through proactive communication
- Deliver concierge-level patient experience
Quality & Program Development
- Participate in protocol refinement and clinical innovation
- Maintain high standards of safety, compliance, and documentation
- Contribute to outcome tracking and program optimization
- Support performance-driven, data-informed care models
Qualifications
- Licensed Nurse Practitioner (NP) or Registered Nurse (RN) in good standing with the College of Nurses of Ontario (CNO)
- Minimum 5 years of relevant clinical experience
- Strong clinical skills: injections, IV therapy and phlebotomy skills
- Experience performing women's health procedures (desired not required)
- Experience or demonstrated strong interest in:
