Job Summary
The Homeshare Provider at Connect Community Living (CCL) is responsible for offering a safe, supportive, and nurturing home environment for an individual with disabilities. This role includes providing day-to-day support, fostering independence, promoting community engagement, and ensuring the individual’s physical, emotional, and social well-being. Homeshare Providers play a critical role in enabling individuals to live fulfilling, integrated lives within the community while adhering to CCL’s person-centered and inclusive approach to care.
Key Responsibilities
1. Home Environment and Safety
- Create a welcoming and safe home environment that promotes independence, comfort, and respect for the individual’s personal space and preferences.
- Ensure the home is maintained to safety standards, identifying and addressing potential risks to prevent accidents or injuries.
- Follow CCL’s safety protocols, including emergency procedures, fire safety, and crisis response planning.
2. Personalized Support and Daily Living Assistance
- Assist with daily living activities as needed, including personal hygiene, dressing, meal preparation, and household chores.
- Follow the individual’s Personalized Support Plan (PSP), encouraging skill-building, personal growth, and participation in daily routines.
- Foster independence by providing appropriate guidance and support while respecting the individual’s autonomy and choices.
3. Community Integration and Social Engagement
- Support the individual’s engagement in community activities, recreational outings, and social events to promote inclusion and personal interests.
- Facilitate opportunities for the individual to build social connections, develop friendships, and participate in meaningful activities.
- Collaborate with CCL and community partners to expand access to resources, programs, and services that align with the individual’s goals.
4. Health and Wellness Support
- Monitor the individual’s health and well-being, administering medications as prescribed and scheduling healthcare appointments when necessary.
- Support health and wellness goals by encouraging nutritious meals, exercise, and healthy lifestyle choices.
- Document and report any health changes, incidents, or concerns to the Homeshare Coordinator to ensure proactive, responsive care.
5. Documentation and Reporting
- Maintain accurate records of daily activities, support provided, medication administration, and progress toward goals in compliance with CCL policies.
- Complete incident reports, behavioral observations, and monthly summaries as required, ensuring timely and confidential documentation.
- Communicate regularly with the Homeshare Coordinator, providing updates on the individual’s well-being, achievements, and any areas of concern.
6. Training and Compliance
- Participate in required training programs, including first aid, positive behavior support, health and safety, and CCL’s Non-Restraint Policy.
- Adhere to CCL’s policies, procedures, and the standards set by Community Living British Columbia (CLBC) for Homeshare services.
- Engage in continuous learning and professional development to enhance caregiving skills and adapt to evolving best practices.
7. Family and Care Team Collaboration
- Communicate and collaborate with the individual’s family, CCL staff, healthcare professionals, and other members of the support team.
- Act as an advocate for the individual, ensuring their preferences and needs are understood and respected by all involved parties.
- Attend support team meetings and contribute to discussions on service planning, goal setting, and the individual’s progress.
Qualifications
- Education: High school diploma or equivalent; additional certification in caregiving, community support, or a related field is an asset.
- Experience: Prior experience in home care, disability support, or a similar role is preferred. Experience in community integration and working with individuals with diverse needs is advantageous.
- Skills & Knowledge:
- Understanding of person-centered support practices and the ability to implement individualized care plans.
- Basic knowledge of health and safety protocols, including first aid and emergency procedures.
- Strong interpersonal and communication skills to engage positively with individuals served, their families, and the care team.
- Organizational and time management skills to balance daily routines, record-keeping, and community involvement.

Working Conditions
The Homeshare Provider will reside with the individual in their home, providing day-to-day support in a live-in arrangement. Flexibility in work hours, including weekends, evenings, and on-call availability, may be required to ensure comprehensive care.
