The John Howard Society of Belleville is a criminal justice agency that provides services in the areas of prevention, intervention, and reintegration. Our primary focus is to reduce the impact of crime and its causes through the delivery of relevant programs.
The HART Hub is a community-based support center providing coordinated access to health, social, and housing services for individuals experiencing complex challenges. This position is part of a 24/7 HART Hub operation, The Bridge, and requires availability to work days, evenings, weekends, and overnight shifts.
Position Summary:
This position is employed by JHS and functionally reports to the Client Service Specialist at CMHA HPE. The Client Support Worker provides direct support to individuals accessing The Bridge, engaging clients respectfully and non-judgmentally while recognizing the impact of trauma and systemic barriers. The role involves intake, assessment, and referrals; shared case management; group facilitation; and active support with housing, health, and community resources. The CSW also contributes to crisis response, safety, and documentation as part of a 24/7 interdisciplinary team.
Duties and Responsibilities:
Daily Tasks
· Engage clients in a respectful, non-judgmental manner.
· Support intake processes, complete assessments, and assist with referrals.
· Maintain accurate and timely documentation in digital systems (HIFIS, EMHWare, SHIIP).
· Ensure a clean, safe, and supportive environment.
· Apply client-centred and trauma-informed practices in all interactions.
· Participate in cleaning, safety, and upkeep of the service environment (including lifting/moving supplies up to 40 lbs).
Programming and Services:
· Provide direct client support, advocacy, and systems navigation.
· Participate in shared case management and care planning.
· Facilitate groups, workshops, or activities that promote well-being, skill-building, and connection.
· Collaborate with community partners, law enforcement, and first responders as required.
· Apply de-escalation techniques and crisis intervention strategies in emergency situations.
· Contribute to an inclusive, culturally safe, and collaborative environment.
Qualification and Skills:
· Demonstrated knowledge of trauma-informed care, harm reduction principles, and culturally safe approaches.
· Strong understanding of the social determinants of health, and the challenges related to homelessness, mental health, and substance use.
· Ability to work effectively in a fast-paced, dynamic environment, while managing shifting priorities and diverse client needs.
· Experience supporting individuals from a range of backgrounds, including those facing systemic marginalization.
· Excellent interpersonal and critical thinking skills; able to respond with empathy while maintaining appropriate boundaries.
· Comfortable working both independently and as part of an interdisciplinary team.
· Skilled in systems navigation and knowledgeable about local health and social service resources.
· Familiarity with client information management systems (e.g., HIFIS, EMHWare, SHIIP) considered an asset.
· Experience with group facilitation or workshop delivery is an asset.
· First Aid/CPR, Crisis Intervention, and Nonviolent De-escalation training (or willingness to obtain certification upon hire).
· Availability to work days, evenings, overnights, weekends, and holidays as scheduled.
