The incumbent will be responsible for supporting, engaging and focusing on the needs of youth being released from custody. Operating within custody corrections, the System Navigator supports youth during the pre-release, transition planning, and release phases. Working in compliance with institutional policies, routines, and security protocols, the System Navigator collaborates with custody case managers, staff, and a range of clinical teams to support release planning and continuity of care. The System Navigator supports youth, in partnership with their families and communities, as they progress toward reunification and reintegration. The position links cultural supports, service providers, and relevant programs to coordinate and support the youth’s healing journey.
Duties:
The System Navigator provides direct, relationship¿based support to youth transitioning from custody to community by assisting case managers to coordinate individualized reintegration planning and connecting youth and families to appropriate community, cultural, and clinical supports. Working collaboratively with families, communities, Elders, Knowledge Keepers, and multidisciplinary service partners, the position supports healing¿centred pathways that promote safety, continuity of care, and positive identity development. The incumbent facilitates coordination among service providers, participates in case conferences, documents youth engagement and progress, and responds to crisis or behavioural situations as required. The role also includes supporting staff through coaching, training, and feedback on reintegration planning, as well as accompanying or transporting youth, parents, or caregivers when necessary.
Qualifications:
- Post-secondary education or training in a social work field and/or an equivalent combination of education, training and experience.
- Experience working in Indigenous and/or Northern communities.
- Experience engaging with Indigenous youth, families, and communities, including demonstrated knowledge of Indigenous teachings, cultures, and histories, and participation in cultural activities, practices, or ceremonies.
- Experience in community service planning, delivery, and coordination, including connecting individuals to appropriate community-based programs and supports.
- Strong organizational and time management skills with the ability to manage multiple priorities
- Excellent interpersonal skills with the ability to work collaboratively with internal and external partners.
- Strong verbal communication skills
- Strong written communication skills
- Ability to work both independently and as part of a team,
- Proficiency in Microsoft Office applications including Word, Excel, PowerPoint, and Outlook.
Desired:
- Experience working from a trauma-informed and strengths-based approach, including supporting youth who may present with complex needs or behaviours.
- Knowledge of, or experience working within, the Wraparound Intervention Model/High-Fidelity Wraparound process.
- Experience supporting or facilitating land-based or culturally grounded programming.
- Ability to communication in an Indigenous language is an asset

- Must provide and maintain a satisfactory Child and Adult Abuse Registry Check
- Must complete and maintain a satisfactory Internal Security Screening Level IV
- Must have a valid Class 5 Driver’s Licence
- Must be able to work shifts that include days, evenings, nights, and weekends is required.
- Must be able to work overtime as necessary.
- Must have the ability to travel to and from remote isolated communities when required.
