We are seeking a dedicated and experienced Primary Health Care Nurse Practitioner to join our multidisciplinary team. Duties
- Conduct thorough patient assessments and develop individualized care plans as per your scope of practice
- Collaborate with our multidisciplinary team when appropriate
- Maintain accurate documentation of all patient interactions in compliance with healthcare regulations.
Requirements
- Valid Nurse Practitioner license
- Experience working independently and willing to work in a multidisciplinary setting
- Strong case management skills with the ability to coordinate comprehensive patient care plans.
- Excellent communication skills for effective collaboration with patients, families, and healthcare teams.
- Ability to handle sensitive situations compassionately and professionally.

Our clinic currently offers Physiotherapy (Pelvic Health, Vestibular, MSK), Occupational therapy, Psychotherapy and Naturopathic Medicine.

Pay: $60.00-$70.00 per hour
