Providence Centre is a licensed supportive living and accredited long-term care facility, originally built in 1965 by the Sisters of Providence. It is home to many retired Catholic Sisters and is located on a serene 25‑acre property featuring beautiful grounds, a chapel, and an oratory dedicated to Mary Mother of Sorrows. Nursing, dietary, housekeeping, laundry, and maintenance services are all provided in-house. We are currently seeking a Casual Licensed Practical Nurse (LPN) to join our care team. The successful candidate will provide high‑quality nursing care to residents and will report directly to the Nursing Manager.
Job Status: Casual
Hours of Work: This position includes a mix of day, evening, weekend, and occasional night shifts. Shift times vary and may include 7:00 AM–1:00 PM, 3:00 PM–9:30 PM, and 7:00 AM–3:15 PM as part of a rotating schedule.
Responsibilities:
- Provide resident‑ and family‑centered care using the nursing process (assessment, planning, implementation, evaluation).
- Practice according to the CLPNA Code of Ethics and Providence Centre policies and procedures.
- Support resident choice, participation, and control in their care.
- Identify and report ethical or practice concerns to the RN or Nursing Manager.
- Complete Unusual Incident Reports promptly.
- Supervise and guide Health Care Aides (HCAs) during the shift, including delegating restricted activities and monitoring assigned tasks.
- Assess residents on admission and on an ongoing basis, including health status, care expectations, and awareness of health issues.
- Develop, implement, and update individualized care plans in collaboration with the care team.
- Administer medications safely and document therapeutic responses.
- Provide safe, effective nursing treatments and interventions.
- Document assessments, care provided, and evaluations accurately and in accordance with policy and regulatory requirements.
- Participate in care conferences and interdisciplinary team meetings.
- Contribute to creating and maintaining a home-like environment for residents.
- Communicate relevant resident information to the RN or Nursing Manager.
- Assist with meals, ensuring adequate nutrition and hydration.
- Support residents with physical needs and activities of daily living.
Qualifications:
- Licensed Practical Nurse certificate from an accredited institution.
- Current, active registration with the College of Licensed Practical Nurses.
- Minimum one year of experience in a continuing care or related health-care setting (preferred).
- WHMIS, First Aid, and mask‑fit training are assets.
- Eligible to work in Canada.
- Clear Criminal Record Check and Vulnerable Sector Check.
- Up‑to‑date immunization records.
