Overview:
The Hospice RPN is responsible for the provision and management of cost-effective nursing care to the clients at Carefor Hospice Cornwall. Care is provided within the framework of quality & risk management and the standards and scope of practice set by Carefor and the College of Nurses of Ontario. The nurse is responsible to teach and delegate tasks to different levels of workers, including Personal Support Workers and volunteers. The nurse creates an environment of practical, psychosocial and spiritual support for the clients of the Carefor Hospice Cornwall.
Responsibilities:
- Provides complete, relevant assessment of the client, following pre-set procedures

Implements the care plan based on the nursing assessment in collaboration with the Hospice RN or physician

Implements nursing strategies or interventions according to the established care plan

Performs technical nursing skills at a safe level in accordance with scope of practice, nursing principles and Carefor policies and procedures

Describes, evaluates and documents client’s responses to the care plan utilizing the electronic record

Utilizes appropriate interviewing and communication skills to obtain information from the client/family

Describes and documents nursing processes clearly, concisely, accurately and appropriately, according to Carefor standards
- Evaluates client’s responses to care plan for the achievement of realistic client-centered outcomes. Revises plan as required

Demonstrates respect for client’s choices and decisions

Utilizes adult learning principles and established teaching materials to provide health teaching to clients, unregulated care providers and volunteers

Reports and discusses client's status on a regular basis with other members of the health care team

Liaises with the Hospice staff.

Works closely with the PSW’s and volunteers, providing them with direction, teaching and support

Orders and checks new medications from the pharmacy and lists the medications on the MAR sheets
Qualifications:
- Licensed as a Registered Practical Nurse in the province of Ontario

Clinical practice experience mandatory

Experience in community health preferred

Palliative Care mandatory

Excellent skills in assessment, risk identification and management

Collaboration, consultation and counselling to resolve issues

Enhanced chronic disease monitoring and management

Health teaching

Excellent verbal and written communication skills

Ability to work with regulated and unregulated care providers and volunteers

Strong commitment to and empathy with palliative clients and appreciation for the physical, social, psychosocial and spiritual aspects of end of life
