Program Overview
The Eastern York Region North Durham (EYRND) Ontario Health Team (OHT) launched the Seniors Home Support (SHS) Program in 2021, an integrated primary care team serving frail, homebound seniors in the communities of Markham and Stouffville.
The program serves a roster of more than 300 patients who experience physical, cognitive, and social barriers to attending office-based care. Through its current model of care, patients receive longitudinal primary care, including routine and urgent home visits, virtual care, and palliative care. Improving access to in-home primary care is critical for this population: 50% live with significant cognitive impairment, 26% have a history of stroke, and 27% live with moderate to severe heart failure.
The interprofessional team includes family physicians, a nurse practitioner, a pharmacist, a chiropodist, registered nurses, community paramedics, and home care coordinators from partner organizations, including Markham Family Health Team (FHT), Markham Stouffville Hospital (MSH), York Region Paramedic Services Community Paramedicine Program, and Ontario Health atHome. The SHS program is one of Ontario’s few home-based primary care programs that integrate community paramedics as part of the core care team.

Position:
We are presently seeking an enthusiastic individual to join our high functioning interprofessional team as a full-time Nurse Practitioner.
● We offer a competitive compensation package. Remuneration is in line and up to date with the Ministry of Health's recruitment and retention strategy for NPs in a FHT setting.

Reporting to the Executive Director, the incumbent will work within our Seniors Home Support Program serving the primary care needs of homebound seniors. The Nurse Practitioner will assume primary care responsibility for a roster of patients, as well as a supportive role within the collegial interdisciplinary team, providing clinical cross-coverage for team physicians and nurses. The Nurse Practitioner will primarily deliver care via home visits and virtual appointments.
Qualifications and Requirements:
● Current registration with the College of Nurses of Ontario as an RN (EC), in Primary Care
● Member of RNAO or NPAO with professional liability insurance
● Masters of Science in Nursing (MScN) or Masters of Nursing (MN)
● Successful completion of a recognized Primary Care or Adult Nurse Practitioner program
● Basic CPR
● Experience in geriatrics is required
● Experience in palliative and end of life care an asset
● Demonstrated knowledge and experience with promoting evidence-based practices and developing and implementing innovative patient care practices
● Demonstrated clinical leadership within an interprofessional team
● Proven team player
● Ability to work in a fast-paced environment and adapt to evolving program expectations
● Excellent written and oral communication skills
● Ability to work with a diverse patient population staff and patients
● Valid Driver’s License, and access to personal vehicle

Pay: $120,000.00-$125,000.00 per year
