Mental Health Case Worker
3 – Full-Time, Permanent (35 hours per week)
Reporting To: Interprofessional Primary Care Manager
Purpose:
The Scarborough interprofessional primary care access model is a partnership to enhance care for the vulnerable, complex clients living in the Scarborough.
SCHC operates with an integrated and interdisciplinary approach. All roles collaborate with and contribute to different teams and programs to support the community's needs.

Key Areas of Responsibility:
The Mental Health Case Worker (MHCW) provides case management for clients with diverse health, social, and psychosocial needs. The worker facilitates the ongoing assessment, service plan, referral coordination and discharge plan for the client and their family. The MHCW assists the client and family with the goal of maximizing client empowerment and health through culture sensitive service, education and programs both within the program and in conjunction with other service providers.
· Conduct comprehensive client assessments that consider mental health, physical health, social, financial, and environmental needs that supports the client’s recovery.
· Develop, implement, and monitor individualized service and care plans in collaboration with clients, families, and interdisciplinary teams.
· Facilitate referrals and linkages to community resources, healthcare providers, social services, housing, employment support, legal aid, and other relevant services.
· Provide advocacy and guidance to clients navigating complex health and social systems.
· Support mental health needs as part of a broader case management approach, including crisis intervention, psychoeducation, and resource coordination.
· Work with families and caregivers to promote mutual support, capacity building, and empowerment.
· Identify and address barriers that affect access to services, including language, cultural, and systemic barriers.
· Participate in community outreach, engagement, and education initiatives to enhance access to services and culturally responsive care.
· Maintain accurate, timely, and confidential client records and documentation in compliance with organizational and regulatory requirements.
· Participate in team meetings, case conferences, and professional development opportunities.
· Uphold SCHC’s commitment to equity, inclusion, cultural competence, and anti-oppressive practice.
Educational and Professional Qualifications:
· Diploma or degree in Social Service Work or related field.
· Registration with OCSWSSW.
· Minimum 1–3 years of experience in case management, client advocacy, or front-line work with vulnerable populations.
· Demonstrated ability to provide holistic, recovery-oriented, and client-centered care.
· Knowledge of broad determinants of health and the social factors affecting low-income, multilingual, racialized, and LGBTQ2S+ populations.
· Excellent interpersonal, communication (written and verbal), problem-solving, organizational, and time management skills.
· Ability to work independently and collaboratively in a fast-paced, dynamic environment.
· Second language relevant to priority populations in Scarborough (e.g., Chinese dialects, Tamil, Urdu, Arabic, Farsi) is an asset.
· Current HCP CPR and First Aid certification.
Significant Working Conditions
· Flexibility of hours – evening or weekend work may be required.
· Flexibility of working across multiple sites, including community based locations.
· Valid driver’s license and access to a reliable vehicle are required.
· Perform other related duties as required to support client care, program delivery, and organizational priorities.
Remuneration: $30.55 - $37.45
Pay Band: E
