Job Summary
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in our PCC patients. This role requires a strong focus on pain management, ensuring that patients receive comprehensive assessments and appropriate treatments. The Nurse Practitioner will work collaboratively with other healthcare professionals to deliver exceptional patient outcomes.
Responsibilities

Perform comprehensive patient assessments and develop individualized treatment plans.

Order diagnostic tests, prescribe medications, and administer pain interventions within their scope of practice.

Work collaboratively with physicians, specialists, and other healthcare professionals to ensure seamless care for patients with acute and chronic pain conditions.

Educate patients on pain management strategies, lifestyle modifications, and preventive care.

Support mental health needs through assessment, diagnosis, and management.

Provide holistic care that considers the physical, emotional, and psychological well-being of each patient.
Experience

A valid Nurse Practitioner license is required.

Active registration with the College of Nurses of Ontario (CNO).

A minimum of one year of clinical experience in primary healthcare, urgent care, or outpatient settings.

Previous expertise in pain management, chronic disease management, family medicine, or Musculoskeletal Disorders preferred

Familiarity with medication administration protocols and pain management techniques is essential.

Strong communication skills to effectively interact with patients, families, and healthcare teams.

Ability to work independently while also being a collaborative team player.
