POSITION SUMMARY:
Reporting to the Director of Care, the Health Care Aide (HCA) provides on-going client care through direct observation, monitoring and reporting on client status including physical, mental, emotional and psycho-social status. The Health Care Aide’s (HCA’s) role is to fulfill the physical, intellectual, emotional, social and spiritual needs of clients and in accordance with facility policies, procedures and work routines. The Health Care Aide (HCA) is responsible and accountable for providing optimal care and support to clients, client’s families and volunteers using a harm reduction approach.
KEY ACCOUNTABILITIES (INCLUDE BUT NOT LIMITED TO):

Provide cognitive, social, emotional, and psychological support to the client and family.

Provide meaningful and personalized support for clients in the facility as per their customized care plan.

Observe, report, and record physical or cognitive changes with clients to the Licensed Practical Nurse.

Observe and report on client's care/service utilizing appropriate TPFA forms and tools.

Promote and support Individual rights and facilitate meaningful community inclusion.

Facilitate ongoing learning opportunities for personal growth of individuals.

Facilitate opportunities for the development of personal relationships.

Contribute to the development and implementation of personalized plans.

Assist clients with activities of daily lives and personal care which includes but not limited to bed making, oral hygiene, bathing, toileting, dressing, skin and nail care.

Ensure clients needs are met according to their care plan.

Assist with transfer of clients using the appropriate machines such as lifts, wheelchairs, and walkers.

Assist with meals by feeding, serving, and monitoring intake.

Charting client information daily, weekly, or monthly in the Point Click Care module of Point of Care.

Encourage clients to socialize and join activities by reminding them of programs.

Maintain safe and clean work areas, safe and clean environment, including client's equipment, belongings, and nursing equipment.

Maintain confidentiality regarding client information.

Respond to client/family concerns and ensure appropriate action is taken. When necessary, discuss concerns and complex issues with supervisor or decision-making authority.

Promote positive behaviour and support effective communication within the team.

Attend in-services as required.

Serve on the various committees responsible for client care.

Complete annual and mandatory education/in-services as well as all required education/inservices as per organizational requirements.

Other duties as assigned.
EDUCATION AND EXPERIENCE:

Completion of Health Care Aide Certification from a recognized institution in Alberta is strongly preferred.

Courses or training completed in Harm Reduction is preferred.

Active directory status on the Alberta Health Care Aide Directory.

Demonstrated experience working with persons with mental illness and/or addictions is highly desirable.

Previous experience working with seniors is considered a strong asset.

CPR - Health Care provider’s level from Heart & stroke Foundation.

Experience working with RAI MDS 2.0 and Point Click Care (PCC) software is highly preferred.
WORKING CONDITIONS:
This industry requires empathy for and an understanding of the needs of seniors and requires a complete and current (within six months) Vulnerable Sector Check including a Criminal Background Check. This position is a fast-paced, challenging, working environment that provides complex care to seniors and other marginalized populations. The ability to be flexible to meet the demands of this work environment is required for success. This position works closely with seniors daily which requires a high threshold for patience and compassion. This work can be stressful and you may encounter challenging behaviours including physical or verbal aggression. This position does require shift work.
