Regional Integration Lead (health-related manager position), Ontario Osteoporosis Strategy

Osteoporosis Canada (OC), a registered charity, is the only national organization serving people affected by osteoporosis. OC works to educate, empower, and support individuals and communities on bone health and in the risk-reduction and treatment of osteoporosis.
Osteoporosis is a highly debilitating condition in which bones become less dense and fracture more easily. It is the single most important cause of fractures among men and women over the age of fifty, and is a highly prevalent and costly condition. One in three women and one in five men will experience an osteoporotic fracture in their lifetime.
Osteoporosis Canada, with funding from the Ontario Ministry of Health, leads the implementation of the Ontario Osteoporosis Strategy (OOS). The mandate of the OOS is to reduce morbidity, mortality, and costs from osteoporotic fractures using a patient-centered, multidisciplinary approach that is integrated across health care sectors (osteostrategy.on.ca (about: blank)).
Committed to improving health outcomes for people with osteoporosis, or at risk for, osteoporotic fractures, Regional Integration Leads are expected to incorporate best practices in knowledge translation and implement OOS initiatives through collaborative partnerships in communities across Ontario to achieve OOS objectives.
Reports to: Director, Ontario Osteoporosis Strategy
Responsible for:
- Achieving identified OOS goals and objectives within a designated geographical area.
- Implementing the activities outlined in key areas including Fracture Prevention, Healthcare Professional Education and Outreach, and Communications and Patient Education, by fostering collaboration among stakeholders, coordinating regional initiatives and developing and disseminating educational resources.
- Overseeing the Fracture Screening and Prevention Program, a secondary fracture prevention program, in select Fracture Clinics across the region. This includes supporting and managing a team of Fracture Prevention Coordinators.
- Regional Integration Leads are responsible for managing their home office including the maintenance of education resources and office supply inventory.
Duties include:
- Ongoing environmental assessment of fracture prevention/management programs, related programs and services, as well as status of home, community and long-term care institutions in the region.
- Building and maintaining partnerships with hospitals, primary care providers, osteoporosis specialists, long-term care, community organizations, and public health professionals to advance fracture risk reduction initiatives.
- Ongoing evaluation of regional programs and projects to ensure alignment with OOS/Ministry of Health priorities.
- Communicating clinical best practices, as indicated in Osteoporosis Canada’s Clinical Practice Guideline (CPG), through knowledge translation strategies across the continuum of care, including:
- Integration of post-fracture care to provide enhanced care. This includes implementation, development and management of the Fracture Screening and Prevention Program (FSPP) to improve fracture risk assessment, diagnosis of osteoporosis and reducing the risk of future fractures;
- Health care professional education and support through distribution of tools and resources for physicians and allied health care professionals, promotion and adoption of CPG and dissemination of education material for patients/clients;
- Supporting community-based patient education initiatives aimed at reducing the risk of osteoporotic fractures.
- Other duties as assigned.
Qualifications/Experience:
- Post-secondary education, preferably in a health-related field
- At least five years’ experience in developing and managing programs in the health care sector
- Forward thinking self-starter who demonstrates the ability to take initiative and to manage projects independently
- Excellent stakeholder management and relationship building skills in diverse settings
- Team player with superior communications and interpersonal skills
- Staff management experience
- Proficiency with Office 365 and online collaborative, meeting and communication tools
- Knowledge of Ontario’s healthcare system and regional health structures
This is a remote position service Hamilton and surrounding areas. The successful candidate will work from a home office and must have access to a reliable vehicle for frequent travel within the region. This role is offered as a one-year fixed term contract.

Salary: $75,000 per annum
