JOB SUMMARY
The Health Care Aide provides personal care to assigned residents under the direction of the Licensed Practical Nurse in accordance with the care plan.
The HCA provides support, advocacy and choice to Residents and their families to ensure the highest quality of resident focused care achievable within the values of Golden Age Manor. Demonstrates ongoing competence utilizing knowledge, skill, judgment, and evidence-based practice in caring for the residents. Ensures compliance with continuing care health service standards, employment and safety-related statutes, legislation, codes, regulations, and GAM policies. Provides support to the residents and families in an environment that promotes a state of wellness, independence and person-centered care.
KEY RESPONSIBILITIES
- Provides competent, direct personal care to the assigned residents. This includes all dietary duties as assigned to ensure the nutritional needs of residents are being met.
- Provides safe medication assistance of regularly scheduled medications to their assigned residents.
- Provides light housekeeping, laundry, and dining room duties as assigned.
- Participates in the development, implementation, evaluation and revision of the resident care plan.
- Carries out all required activities as assigned to provide holistic care to residents such as recreational duties.
- Observes the residents’ physical condition, behavior and mood and promptly reports any changes in the resident's status to the LPN on shift.
- Ensures effective, timely documentation in accordance with organizational, legislative, and legal requirements.
- Participates in the development, revision and implementation of operational policies, procedures, and programs in collaboration with appropriate team members.
- Participates in the development of plans, objectives and strategies for care services, which are aligned with overall organizational strategic directions.
- Complies with the organizational, policies and procedures, continuing care health service standards, accommodation standards, and relevant legislation and regulations.
- Reports all concerns and complaints from residents, families, and staff to the LPN in accordance with the concern resolution process.
- Responds and reports all unusual incidents and critical, incidents on the assigned unit to the LPN. Initiates appropriate corrective action within the scope of practice to address the immediate concern.
- Reports staff performance concerns to the LPN or Nursing Manager.
- Participates in cost effectiveness of resources on assigned unit or site.
- Promotes a safe work environment for residents, family members, co-workers, and self.
- Reports unsafe conditions within the living and working environment. Takes required action to decrease the risk of injury immediately and/or notify the LPN and/or Nursing Manager of the concern.
- Represents and participates in mandatory corporate or site committees and meetings as appropriate.
- Participates in performance measurement to achieve continuous quality improvement.
- Maintains an ongoing program of personal learning and attends GAM mandatory education sessions in order to maintain own competence. Seeks to benefit from opportunities for professional development and continuing education.
- Acts as a mentor and coach to staff.
- Actively supports, promotes, and implements the values and strategic directions of GAM.
JOB SPECIFICATIONS/QUALIFICATIONS
Education Requirements
- Graduated from a Health Care Aide program recognized by Alberta Health Services.
- Must have completed a recognized Medication Assistance Course.
Minimum Qualifications
- Must be registered with the Alberta Health Care Aide Directory.
- Experience working with the elderly, chronically ill, mental health or the disabled is preferred.
- Completed vulnerable sector check report that is dated within the last 6 months.
Knowledge
- Demonstrated knowledge and commitment to person-centered care.
- Awareness of accident and injury prevention, hazard identification and control, adherence to safe work practice and procedures is essential in this position.
Skills and Abilities
- Ability to read and write fluently in the English language.
- Demonstrated skills in communication, teamwork and customer service.
- Strong organizational skills and the ability to multitask is essential in this position.
